Use this form for chart review after observation is complete. List each medication ordered for the patient from admission until the
time period of the observation. Compare the medications ordered with medication administration actually observed and code
discrepancies in the “Error” field of the MedAdmin automated Excel data submission form. Any medications ordered but not
administered will be added to the file submitted to CALNOC and coded as “omission”, “wrong time,” etc. as appropriate. See
