Tatts EGM Change Form
Form Instructions
1. This form is for requesting changes for a single EGM and/or an existing game. For changes to multiple EGMs you need to use
the Tatts EGM Movements Form.
2. Refer to the Tatts Game Reference List for information on current EGM Games and to locate the Tatts Gameset number.
3. If you are changing an EGM Game that is not on the Tatts Game Reference List then you must complete the Tatts New Game
Form and send with this form.
4. If you are changing a jackpot or EGMs in a jackpot you must complete the Tatts Jackpot Change Form and send with this form.
5. When you have completed this form, send it with any attachments to Intralot at [email protected] or fax 03 9673
3999 ensuring that you have given notice of at least 20 days.
Warning: If you are changing a QCOM EGM with and EGM Triggered Jackpot (Cash Express, Thunderheart, Grand central) the
EGM may need to be master reset.
