About Me meeting guidance (7)

All About Me meeting guidance for settings
The All About Me meeting must be held within four weeks of the EHC Panel agreeing to a
statutory assessment. The person responsible for pupils with SEN in the educational setting
is responsible for convening, chairing and recording this meeting i.e. the SENCO. The
SENCO will usually be the key contact, but if they are not, then the SENCO should liaise
closely with the key contact about this meeting. If the child/young person is not in an
educational setting then the key contact is responsible for organising this meeting.
The All About Me meeting is a person centred meeting focussing on the child/young person
with their parents, in order to capture their views, aspirations and goals for the future. During
the meeting the child/yp’s One Page Profile will be completed and the outcomes sought for
the child/yp should also be captured. It is suggested you look at the questions with
parents/young person before the meeting in order to inform the discussions. Further
information about conducting person centred reviews can be found on the Helen Sanderson
website at helensandersonassociates.co.uk
The family/ child/yp can choose who they wish to be invited; however it is important that
everyone who is involved with the child/yp is invited to the meeting. It may not be possible
for all those invited to attend on the specified date, however the meeting must be completed
within the four week timescale.
In order to meet EHCP timescales the All About Me documentation must be completed and
emailed in electronic format to [email protected] within this four week
period.
All About Me meeting checklist
Welcome and introductions. Apologies.
Explain the purpose of the All About Me meeting:
 To discuss the aspirations and goals for the child/yp
 To complete the One Page Profile about the child/yp
 To agree on the outcomes to be worked towards for the child/yp,
 To make suggestions about the provision required to meet the
outcomes
 To record any further new information about the child/yp
1.History
Make a brief summary of the educational settings attended by the child/yp,
along with dates if known; significant involvement with outside agencies and
details of any moves outside the local area.
2. Complete the One Page Profile with the child/yp and their parents/carers.
This can be drafted before the meeting with the child/yp if necessary, and then
finalised at the meeting. If the One Page profile does not contain the views of
the child/yp then please record whose views it does reflect.
A completed example of a One Page Profile is attached at the end of this
guidance.
3.Suggested outcomes and provision
Look at the outcomes suggested by the professionals as part of the submission
to the EHC Panel.
Are there any other outcomes which have not been suggested?
Agree with parents and child/yp which outcomes should be prioritised over the
current and/or next key stage.
Notes on outcomes:
 Outcomes should always enable children/young person to move
towards the long term aspirations of employment or higher education,
independent living, community participation and good health
 An outcome can be defined as the benefit or difference made to an
individual as a result of an intervention. It should be personal and not
expressed from a service perspective; it should be something that
those involved have control and influence over.
 It should be specific, measurable, achievable, realistic and time bound
(SMART)
 Long term aspirations are not outcomes in themselves
 Outcomes should consider what is important to the child/young person
and what is important for them
 Outcomes will usually set out what needs to be achieved by the end of
a phase /key stage of education
 It is useful to express outcomes in terms of what the child/young person
will be able to do, eg “John will be able to initiate a conversation with
his friends” “John will be able to remain on task for at least 15 mins”
Provision to meet the outcomes should be suggested in terms of
programmes, interventions, strategies, support.
Tick when
completed
4. A Personal Budget does not have to be discussed at this meeting; however
parents may make initial expressions that they are interested in having a
personal budget.
Requests for social care personal budgets should be made through the CAF
process or directly through the social worker (where appropriate). Families can
also self-refer - information about this is on the Local Offer website
www.bwd-localoffer.org.uk (search- Social Care – Referral for Assessment
Pathway, under the Information and Guidance tab) or by telephoning 666400.
Parental queries at this stage around personal budgets for health should be
directed to [email protected].
For queries about personal budgets for education, then the LA would need to be
satisfied that this is provision that could not be provided by or commissioned by
the school or the LA. The provision would also need to have a firm evidence base
that it is necessary for the child/yp ie. there should be reference to it in a
professional’s report. The headteacher would also have to agree to the use of a
personal budget. Further discussions about personal budgets for education would
need to be directed to your SEN Officer.
5. Some children/yp may require a change of school, particularly those who
require a change of provision eg from mainstream to specialist provision and
the discussion around this needs to be recorded. This section does not need to
be completed if the plan is for the child/yp to remain in their current placement,
but if the child/yp is due to transition to the next phase of their education then
record what the parental preference is.
6. Record any additional new information that has not already been submitted
through the referral to the EHC Panel. Ensure that parents are happy that their
views have been communicated through the submission to panel, however,
please be mindful of the ‘Tell it once’ approach.
Thank everyone for their input and close the meeting. It is good practice to
share the One Page Profile at this stage with everyone who works with the
child/yp and to consider the short term steps towards the suggested outcomes.
Ensure that the All About Me documentation is emailed to
[email protected] as soon as possible following the
meeting.

need an adult to help me
sort it out.
If someone hurts me it
pushes my big red button
which means I am really
angry. A teacher should
remind to use my calming
strategies - my lego box and
figures.
Allow me time to tell my
version of what happened
Sometimes I take a long
time to eat my lunch and I
don't have time to play at
dinner time.
