Deadline to Complete Steps for $500 Medical Deductible Waiver: September, 30, 2015 For Associates with Health First Health Plans Insurance $500 Medical Deductible Waiver Opportunity for 2016 We know the combination of Healthy Behaviors with Healthy Care can improve the health and wellbeing of those who choose to participate in them. This is why we have created a new program, my HEALTH FIRST, to provide guidelines and activities to help improve the health and well-being of our associates. The goal of the my HEALTH FIRST wellness program is to offer comprehensive programming to support, engage and motivate associates to adopt Healthy Behavior and Healthy Care activities to achieve the highest level of personal health and well-being. This year, Health First associates with Health First Health Plans Insurance have an opportunity to earn a $500 medical deductible waiver with their 2016 premiums by completing the following three steps: 1. Complete a Wellness Assessment 2. Complete two Healthy Care activities 3. Complete three Healthy Behavior activities These steps and all necessary documentation have to be completed by Wednesday September 30, 2015 in order to receive the $500 medical deductible waiver for 2016. All forms for the requirements are in the Member Portal and your information will be compiled as you complete each step. Once you have completed all three steps, print out the summary of the “Go for It!” page as proof of completion and retain for your records. Note: Health First associates hired after June 1, 2015 are not required to complete the three steps to obtain the $500 medical deductible waiver. Those associates will automatically receive the waiver. Step One: Wellness Assessment Go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal to complete a Wellness Assessment. In the Member Portal, click on my Health First and then the my Health First Medical Deductible Waiver Opportunity from the drop-down menu. If you have never logged into the Member Portal, you will need to set up an account to access the my HEALTH FIRST tab. Once you complete the Wellness Assessment, you will receive a Personal Health Report with your Wellness Assessment Score. Biometrics values are not mandatory this year for the Wellness Assessment (you can enter ‘I don’t know’ when completing the assessment). It is highly recommended associates have up-to-date biometrics values. If you choose to obtain updated or new biometric values, visit your Primary Care Provider or a Health First Associate Wellness Nurse who can provide an order you can take to a lab. If you are updating your biometric values, make sure to have labs completed as soon as possible to ensure results are received in time to complete the Wellness Assessment (typically takes about two weeks). The biometric values have to be manually updated in the Member Portal when completing the Wellness Assessment (if you are including them). Once you start or complete your Wellness Assessment, to go on to the next steps, click on ‘Your Resources’ from the top of the homepage, select ‘Wellness Activities’ from the dropdown menu to access the Healthy Care and Healthy Behavior forms and other information. Step Two: Healthy Care To meet the Healthy Care requirement, associates need to complete two of the activities listed below. There are multiple opportunities under each activity and it is noted how many times you can count each Healthy Care activity. Once you complete an activity, you will need to go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal to complete a form. Information needed for the form includes the date and location of the activity and the provider name (form does not need to be signed by the provider). Annual Well-Care Visit Visit your Primary Care Provider for a well-care visit or have a physical completed at a location of your choice. When you visit, you also can request to have your biometric values updated. An annual well-care visit can only be counted one time (even if you have multiple visits). After you have seen your provider or had a physical, go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal to complete the Healthy Care: Annual Well-Care Visit form (click on ‘Your Resources’ from the top of the homepage, select ‘Wellness Activities’ from the drop-down menu to access the Healthy Care and Healthy Behavior forms). Preventive Screening or Exam Have a preventive screening or exam, such as skin cancer check, pap smears, mammograms, colonoscopies or bone density exams. The screenings and exams are counted separately (i.e. if you have a skin cancer check and mammogram, this is counted as two activities). By completing two preventive screenings or exams, you can meet the Healthy Care requirements. Click here (or visit www.myHEALTHFIRST.org) for a list of preventive screenings. After you have received your preventive screening or exam, go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal to complete the Healthy Care: Preventive Screening or Exam form (click on ‘Your Resources’ from the top of the homepage, select ‘Wellness Activities’ from the drop-down menu to access the Healthy Care and Healthy Behavior forms). For preventive screenings and exams not listed, please input your information in the ‘Other’ field when completing your form. Step Two: Healthy Care (continued) Dental Screening or Exam Have a dental screening or exam. A dental screening or exam can only be counted as one activity. After visiting your dental provider, go to www.myHEALTHFIRST.org and log onto the Health First Health Plans Member Portal to complete the Healthy Care: Dental Screening or Exam form (click on ‘Your Resources’ from the top of the homepage, select ‘Wellness Activities’ from the drop-down menu to access the Healthy Care and Healthy Behavior forms). Nutrition Counseling Medical Nutrition Counseling will help you choose the right foods to eat for specific health conditions. Click here (or visit www.myHEALTHFIRST.org) for a list of approved programs. Participating in a nutrition program can only be counted one time, even if you participate in multiple programs. After completing one of the approved programs, go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal to complete the Healthy Care: Nutrition Counseling form (click on ‘Your Resources’ from the top of the homepage, select ‘Wellness Activities’ from the drop-down menu to access the Healthy Care and Healthy Behavior forms). Disease Management If you have been diagnosed with heart failure, diabetes, COPD, coronary artery disease or asthma, you may choose to complete a formal Disease Management program sponsored by Health First Health Plans. Click here (or visit www.myHEALTHFIRST.org) to view these options or to register for a program. Participating in a Disease Management program can only be counted one time, even if you participate in multiple programs. After you complete the program, go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal to complete the Healthy Care: Disease Management form (click on ‘Your Resources’ from the top of the homepage, select ‘Wellness Activities’ from the drop-down menu to access the Healthy Care and Healthy Behavior forms). Step Three: Healthy Behaviors To meet the Healthy Behaviors requirement, associates need to complete three of the activities listed below. There are multiple opportunities under each activity and it is noted how many times you can count each Healthy Behavior. Once you complete an activity, go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal to complete a form (click on ‘Your Resources’ from the top of the homepage, select ‘Wellness Activities’ from the drop-down menu to access the Healthy Care and Healthy Behavior forms). Tobacco Free Go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal to complete a Healthy Behavior: Tobacco Free form and attest to one of the following (counts as one Healthy Behavior): A. Never used tobacco products - OR B. You have stopped using tobacco products including smoking, chewing and/or dipping for at least 12 consecutive weeks during the 2015 calendar year. Online Seminars There will be four online seminars in the Member Portal (a new one is added monthly). Go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal and select the my Health First tab. Select ‘Your Resources’ from the top navigation bar and then ‘Seminars.’ For each online seminar you complete, this counts as one Healthy Behavior activity. By completing three, you will have met your Healthy Behavior activity requirements. Each time you complete one, your information is updated. Each seminar takes about 20 minutes to complete. Formal Fitness Activity Count this Healthy Behavior by participating in a formal fitness activity or routine, such as regular exercise (at least 20 minutes, three days a week, for four consecutive weeks) at a Health First Pro-Health & Fitness Center, other gym location in the network or on your own. You can only count this Healthy Behavior one time even if you exercise multiple times or participate in multiple organized runs/walks (i.e. Running Zone Race Series). Other fitness activity options include participation in fitness activities such as intermural sports, completing a race through The Running Zone or Runner’s Depot, etc. You also can complete activities not listed as long as they meet the criteria of at least 20 minutes, three days a week, for four consecutive weeks. Step Three: Healthy Behaviors (continued) Formal Fitness Activity (continued) Once you complete an activity, go to www.myHEALTHFIRST.org and log into the Health First Health Plans Member Portal to complete the Healthy Behavior: Formal Fitness Activity form. (click on ‘Your Resources’ from the top of the homepage, select ‘Wellness Activities’ from the drop-down menu to access the Healthy Care and Healthy Behavior forms). Wellness Challenges Wellness Challenges are month-long activities that encourage you to work on habits that promote a healthy lifestyle Reach the Challenge goal and earn one Healthy Behavior automatically (your participation is tracked in the Member Portal). You can count each Wellness Challenge one time. By completing three, you will have met your Healthy Behavior activity requirements. June: Let’s Get Moving! Incorporate physical activity into your day. Take small steps throughout the day to get moving. Park away from the entrance, take the stairs instead of the elevator or take a walk with other associates instead of sitting during your break. Earn one point for each day you add physical activity to your day. You need 8 points to count this Healthy Behavior. You can only count this Healthy Behavior one time. July: Dump the Junk Earn one point for each day you go junk food free. You need 20 points to count this Healthy Behavior. You can only count this Healthy Behavior one time. August: Beat the Heat Earn one point for each day you drink at least three eight-ounce glasses of water. You need 20 points to count this Healthy Behavior. You can only count this Healthy Behavior one time. September: Stress Busters Earn up to two points each day by getting six to seven hours of sleep each night and by spending at least 15 minutes meditating or doing another type of relaxation method each day. Reach 40 points to count this Healthy Behavior. You can only count this Healthy Behavior one time.
