Requirements for Carriers
1. CERTIFICATE OF INSURANCE (CARGO & LIABILITY)- CF Paid Carriers Only
49 CFR part 387 subpart A
a. Desire $1,000,000 Liability on all non-hazardous materials but will accept $750,000.
b. $5,000,000 liability for NH3 and $1,000,000 for all other hazardous materials.
c. Cargo. Security required to compensate shippers or consignees for loss damage to property
belonging to shippers or consignees and coming into the possession of motor carriers in
connection with their transportation service, (1) for loss of or damage to property carried on
any one motor vehicle—$5,000, (2) for loss of or damage to or aggregate of losses or damages
of or to property occurring at any one time and place—$10,000.
The certificate needs to show as additional insured:

2. Copy of OPERATING AUTHORITY (DOT/FMCSA or Intrastate)
3. HAZMAT REQUIREMENTS (If hauling Hazmat product)
a. HAZMAT DOT CERTIFICATION, copy of US DOT’s Hazardous Materials Certificate of
Registration.
b. DOT Safety Permit – This is needed if you are hauling Anhydrous Ammonia
i. Also Need statement indicating carrier has an ammonia hose testing program in
place.
