School Choice – Letter of Intent
For Option 5
Twillman
Option 5 would allow for students from Twillman attending Hazelwood East Middle to have a
choice to attend Hazelwood Southeast Middle, in lieu of attending Hazelwood East Middle at the
Hazelwood East Middle/East High Complex.
Please complete the following information and return this document to the school office no
later than April 15th indicating your child’s attendance preference for 2017-2018. If a
letter of intent is not returned indicating choice, the student will be assigned East Middle.

This choice indicated above is binding should the Board of Education approve Option 5 at
the May 2nd Board of Education meeting.
