Tip sheet: MHIS Client ID Numbers
MHIS allows for three types of client IDs to be used to identify clients. There is a hierarchy for
determining which client ID to use, which is as follows:
1. Patient Master Index Number (PMI): for previous and current clients on Minnesota Health
Care Program (MA, MinnesotaCare, GAMC and MCO/PMAP), a PMI number can be obtained from:
o MHCP Member ID Card: PMI is the client’s Member Number on their MHCP Member ID
Card (see example below)

MN-ITS: MN-ITS Eligibility Request look-up feature allows to search for the PMI also called
the Subscriber ID using the client’s date of birth, SSN, or last/first name. Additional

2. Shared Master Index number (SMI): for all clients with a county case. SMI is a unique client
identification number created to cross-reference MAXIS, MMIS, PRISM, and SSIS.
o The SMI ID is only used when no PMI exists
3. Alternative Mental Health Identifier (AMH ID): The AMH ID contains the first four digit
numbers (which identifies the provider– contact the grant consultant for provider number), and
then next four digits is created by the provider which should begin with 0001 for the first client and
then increase by 1 as additional AMH ID are needed. Grantees/Providers should search client list on
MHIS, to determine next number to be assigned to a new client that does not have PMI or SMI. The
provider must link the AMH ID with the client in their system, to use for future continuing and
discharge client status reporting. AMH ID must not contain any part of the client’s SSN.
o Only if PMI and SMI do not exist – use the Alternative Mental Health Identifier (AMH ID)
