Unintentional Release of Regulated Material[s]Log
Note:
(i)
For the purpose of this form, regulated material[s] includes, but not limited to: GMOs and materials subject to quarantine;
(ii)
This form needs to be located within the facility and made available for annual inspections or upon request.
(iii)
All facility users should be aware of unintentional release procedures.
Procedure:
1.
Report unintentional release or spills of any regulated material[s] to your project supervisor, facility manager, or RSTO, and note on unintentional release form.
2.
Please refer to documented facility procedures for spills management information.
3.
The relevant person (e.g. project supervisor, facility manager, or RSTO) should then report this to the UBC secretary, and if/when appropriate submits an incident report using
the University’s online GSafe reporting system.
4.
The University’s Health and Safety Adviser[s] will investigate the incident on behalf of the UBC and contact the relevant regulating body (e.g. OGTR or Department of
Agriculture) if required.
5.
Update risk assessment and procedural information to prevent the similar incident occurring.
