02 - Electronic Transfer Process Detail

New electronic transfer process (see below for directions)
 It will include transports to Mercy and the occasional transfers to a Burn Center or to University for OMFS.
 This process will not cover psych transports/consults for BHC (use the Telepsych process instead)
 Step 1 a-f is for the ED physician to complete
 Step 3 a-b if for the nurse.
Transfers to Mercy
 You will no longer have to ask the secretary to call the PCL to call CHG-Mercy.
 Just use the electronic form.
 General adult medicine is what you would choose for CHG.
 Please use that notes box as much as you can. The more information you give the PCL, the more seamless it will be.
 As soon as PCL receives the request, they will check bed status and then page CHG Mercy, who will call you back.
 MCA is automatically notified of the transport need and will be ready to coordinate once a bed assignment is available at Mercy

Electronic Transfer Request Process Detail – ED
Step 1 (ED):

When a patient needs to be transferred or the provider is requesting a non-telepsych consult, select the Transfer & Transport
Request PowerForm from the FirstNet toolbar (Red Ambulance Icon)

Step 1a (ED):

Complete all the required fields on the form, noting the Blue reference text next to some of the questions for further information:

Step 1b (ED):

In the Request Type question, if “Transfer” is selected, the following questions will then be made available and required:

In the Transport Arrangements Needed question if “Yes” is selected, the Transport Details section will open:


Step 1e (ED):

When all required fields have been filled out, click the green checkmark to sign the Transfer & Transport Request PowerForm.
Step 1f (ED):

When the “Transfer & Transport Request” powerform is signed, a Red Ambulance icon will appear in the Dis Column letting PCL
know that a transfer request has been submitted:
Step 2 (PCL):


When the Transfer Request Icon (Red Ambulance) turns Yellow, PCL has acknowledged that they have received the Transfer & Transport
Request.
PCL is working on the transfer request and may call for more information.

Step 3 (ED):

When PCL completes their steps for the transfer request:

The Yellow Ambulance icon will change to a Green Ambulance icon indicating that the receiving provider and facility/unit
information is now available.

IF the Request was submitted as a “Transfer”, the yellow CMN icon will appear in the Dis column indicating that the Certificate of
Medical Necessity (Transfer & Transport MD Certification PowerForm) needs to be completed.


To complete the CMN, select the Transfer & Transport Request PowerForm from the FirstNet toolbar (Yellow Diamond Icon):

Step 3b (ED):



The transfer details that PCL documented will pull forward into the CMN PowerForm
Review/confirm the information and chart any additional required fields, including the CERTIFICATE OF MEDICAL NECESSITY question,
then sign the form
Once signed, the Yellow CMN icon will disappear from the tracking board and the Green Ambulance Icon (Transfer Complete) will
remain on the tracking board.
