Section B -- MEETING SUMMARY: (Clearly and legibly provide a summary of the meeting)
Introductions were made by Ms. Davis, LEA. The purpose of the meeting is to review the additional information
gathered at the request of the reevaluation team and determine continued eligibility/need for special education
services and the present levels of academic achievement and functional need for John. The team met on
xx/xx/xxxx and determined additional information was needed in the areas of cognitive processing and academic
achievement. John currently receives special education services under the category of speech-language
impairment.
Mr. V, the school psychologist, reviewed the additional assessment data. John's cognitive processing scores were
within the average range in the areas of verbal comprehension and nonverbal reasoning. He had difficulty with
tasks that required short-term auditory memory. The assessment of John's academic achievement on both
standardized and informal measures showed John's basic reading and reading fluency skills were an area of relative
strength. Reading comprehension was an area of significant weakness for him. Math calculation and math
reasoning (when items were presented orally to John) were areas of relative strength. John demonstrated no
problems in the area of written expression.
The team discussed the additional data and determined that John meets eligibility criteria under the additional
category of LD in the area of reading comprehension. John's IEP was reviewed and revised to include additional
accommodations, supports, and goals to address his reading difficulties.

Section C - ACTIONS TAKEN: (Summary from Meeting minutes) As a result of this IEP/Staffing meeting, we

1. As a result of the IEP/Staffing meeting, the IEP/Staffing Team proposes or refuses (circle one) to:
gather additional information to determine continued eligibility for special education and related services under
IDEA and to determine John’s present educational needs.
2. Explanation of why the school district/agency proposes or refuses to take this action:
The reevaluation team has reason to suspect John may have an additional disability under IDEA and
determined that additional information was needed to assist the team in appropriately identifying all of John’s
educational needs.
3. Description of each evaluation procedure, assessment, record, or report the school district used in deciding
to propose or refuse the action:
The team reviewed information from records, current classroom performance, previous state and district
assessment, and input from parents and teachers as well as John’s current IEP.
4. Description of any other choices that the IEP team considered and the reasons why those choices were
rejected:
The team considered John’s current classroom performance and level of support provided by his current IEP
and determined that his current IEP may not be meeting his needs.
5. Description of any other reasons why the school district proposed or refused (circle one) the action:
The team did not have enough existing information to determine John’s specific educational needs or make any
appropriate modifications to John’s current IEP. The team requested additional information in the areas of
cognitive processing and academic achievement.
