SRU Men’s Ice Hockey Team Player Contract
Acceptance: We (player/parent) the undersigned, do hereby agree:
• To play for the Slippery Rock University Men’s Ice Hockey Club Team (ACHA Div. 1) for the
2017-2018 Hockey Season
• That the date of birth provide is true and correct
• To abide by the player and parent conduct rules, locker room safety policy, SRU bylaws and the
bylaws of ACHA and USA Hockey
• To be available for ALL practices, scrimmages, games, tournaments, team meetings, and team
activities. Players will contact the coaching staff or team manager in the even of a scheduling
conflict with any of the above.
• To support and participate in team/charity fundraisers
• Agree to pay $3200 in player fees, of which $2000 will be due prior to the player being able
to participate in try-outs and the remaining $1200 will be due by December 1, 2017.
• The walk on fee will be $75.00; due before the player will be allowed to try out.
• A payment schedule will be provided for each player, the player’s parents/guardians will be
responsible for keeping all players cost current. Parents/Players understand that all funds are
non-refundable. If deferment is needed, please contact the Team Advisory Board immediately.
• NSF and returned check charges of $35.00 will be charged for any check returned to the
organization. Failure to make prompt payment of all monies due will result in suspension of the
player until payment is made.
• Player must be enrolled in a Baccalaureate Program at Slippery Rock University as a full-time
student with a minimum of 12 credits per semester; must complete 24 credits per Academic year.
• Scholastic Guidelines: Must maintain a GPA higher than 2.0
• Zero tolerance of physical abuse, verbal abuse, or threats of any kind shall not be directed at any
on or off ice officials, rink staff, tournament officials, players, coaches, parents or fans. This
includes but is not limited to shouting at referees, or taunting opposing teams parents or fans. In
general, any conduct unbecoming of an amateur athletic event is strictly prohibited.
• USA Hockey Registration & Insurance; see USA Hockey Guidelines.
• Players must be in good standing with the ACHA, players will not be permitted to play until they
are in good standing.
• Locker Room Guidelines: Proper etiquette at all times.
• Must sign Code of Ethics, etc. from USA Hockey website
• Behavior such as fighting fouls language, and stick banging, or excessive penalties will not be
tolerated.
• Team email lists are to be used for team business only. This information is provided to facilitate
the communication of team members, coaches, and parents. Distribution of this confidential
information to outside sources is forbidden.
• Player equipment: Players will provide much of their own equipment. All equipment provided by
the team will be returned at the end of the year during the season-end exit interview. Failure to
do so will result in fines and/or magistrate prosecution.
• Dress code and arrival times: the Head Coach will determine the team dress code. The
coaching staff & team management will determine arrival times for games, practices, and other
team functions. Failure to comply will result in appropriate disciplinary action.
