Mid-Valley Behavioral Care Network administers mental health and chemical dependency services on behalf of Willamette Valley Community Health Coordinated Care Organization (WVCH CCO) Primary Behavioral Health in Primary Care (PCBH); Mental Health and Substance Use Services Provided in Primary Care Specialty Mental Health Outpatient Services Specialty Substance Abuse Programs Hospitals, Community, Organizations The role of a PCPCH includes coordinating care across settings, including patients with mental health needs that must be co-managed and transitioning care from specialty when patients are stable enough to be managed in primary care Primary Care Provider is the first line of behavioral health care. They conduct screening, manage some medications and support stable behavioral health patients while also managing their medical conditions with the support of care team members like medical assistants, care coordinators and care managers. PatientCentered Primary Care Home (PCPCH) Behaviorists are primary care team members who receive warm handoffs and assist in assessing and managing the emotional/behavioral aspects of chronic disease self-management and preventive care. They provide brief, targeted interventions and can also manage referrals and collaborate with specialty behavioral health services. Co-located specialty mental health Medical clinics can employ and co-locate behavioral health staff to provide specialty care to their patients, and become part of the MVBCN panel of providers. Secondary Primary Care directly refers patients to services or patient initiates care—no referral needed Some patients need mental health and/or alcohol & drug treatment services beyond that which can be offered in the primary care setting. Specialty mental health and substance abuse treatment programs in the community provide a continuum of services that address mental health, addiction and recovery issues for children, adolescents and adults.
