The role of a
PCPCH includes

Primary Care Provider
is the first line of behavioral health
care. They conduct screening,
manage some medications and
support stable behavioral health
patients while also managing their
medical conditions with the support
of care team members like medical assistants, care coordinators and care
managers.

Behaviorists
are primary care team members who
receive warm handoffs and assist in
assessing and managing the emotional/behavioral aspects of chronic disease self-management and preventive
care. They provide brief,
targeted interventions and can also
manage referrals and collaborate with

Co-located specialty mental health
Medical clinics can employ and co-locate behavioral health staff to provide specialty care to their patients, and
become part of the MVBCN panel of providers.

Secondary
Primary Care directly refers patients to services or patient initiates care—no referral needed
Some patients need mental health and/or alcohol & drug treatment services beyond that which can be offered in the primary care setting. Specialty mental health and substance abuse treatment
programs in the community provide a continuum of services that address mental health, addiction and recovery issues for children, adolescents and adults.

Tertiary
Some people have such serious mental health problems they require inpatient care. Examples of patients who would require this level of care are those with psychotic disorders who have voices telling
them to harm others and they are at risk for doing so, patients with life‐threatening withdrawal symptoms, patients who are at imminent risk for self‐harm, etc.

Obtaining Pre-authorization
Screening for inpatient and diversion services is done by crisis teams in each county. They can also authorize hospital
diversion services and provide short term interventions and linkage to outpatient and community services.
