What are the aims of
the Home Treatment Team?
We aim to offer an alternative to inpatient
hospital care. We will work together to plan
your care and recovery.
We will:
• work closely with you so you can be treated
in your own home
• ensure your care is planned and agreed with
you
• promote your independence.
Who is the service for?
The service is for people aged between 18 and
65 living in the borough of Croydon (or over 65
if already in touch with a community mental
health team). Our service is for anyone who is
mentally unwell and needs more help from a
Home Treatment Team.
How can I be referred?
We accept referrals from community mental
health teams, doctors, approved mental health
practitioners, nurses, occupational therapists,
psychologists, inpatient wards including
Gresham 1, Gresham 2, Foxley Lane Womens'
Service and Ashburton Road. We don't accept
direct referrals from patients, GPs or the police.
What can you expect?
The Home Treatment Team consists of
psychiatrists, nurses, approved mental health
practitioners, support time and recovery
workers and an administrator. Your care will be
planned and agreed with you.
The first visit is carried out at your home and we
will agree with you there how often a member
of the team will visit you. You can have a
member of your family or a carer present if you
wish. Although the first visit is at your home,
future meetings can be planned at other
locations, for example in a library, a coffee shop
or a park. We cannot confirm that it will be the
same member of the team visiting you each
time, but all members of the team will be fully
updated with your ongoing care.
You will have daily contact with the team until
it has been agreed for your care to be
transferred. This may be to an existing team, for
example your community mental health team or
if ongoing care is not needed you will be
discharged back to your GP.
Medication will play a part in your care and we
will discuss this with you at your medical
review, which is when you can meet a doctor
from the team.
We are a 24 hour, seven days a week service.
The office is physically staffed from 8am to
10pm.
Croydon University Hospital A&E carries out
assessments on our behalf after 10pm. We will
not accept verbal abuse and threatening
behaviour. Action will be taken including
criminal prosecution to ensure our services are
safe and continue to provide a high quality of
care.
We always welcome your feedback on our
services.
You can give feedback either in writing by
sending a letter to the team leader at the team
address or you are welcome to speak directly to
a team member or the team leader by
telephoning the team base. You can also give
feedback at each contact with a team member.

the above hours please contact your GP or the
Emergency Department (ED) at Croydon
University Hospital (020 8401 3000).
