This decision tree is designed to assist you in determining the Date of Receipt of Referral. The Indication for Colonoscopy is a data classification for submission to CCO, and not for the purposes of triaging patients. Please submit only the main Indication for Colonoscopy following this Indication Order. Patient Entry Point Indication Order Date of Receipt of Referral (Submit Main Indication in the following order) In-Patient: Date of Admission Large bowel symptoms May include: rectal bleeding, altered bowel habits, or pain A polyp was taken in a previous colonoscopy Patient has/had a 1st degree relative with colorectal cancer. CRC Screening Program Eligibility: Screening to start when patient turns 50 or 10 years before the age of the 1st degree relative when diagnosed with colorectal cancer (which ever is first) Patient had a positive FOBT result. CRC Screening Program Eligibility: Men and women age 50+ with Positive FOBT No other indication for a colonoscopy. May include: Patient Initiation 1. Symptomatic (SA) 2. Surviellance (CN) 3. First Degree Relative (FD) Out-Patient: Earlier of the Date the Referral was received by the: ● Facility ● Specialist (Endoscopist/Surgeons) Last Colonoscopy > 12 months: More recent of: ● Date of receipt of referral (at either Facility or Specialist) ● Date of last Colonoscopy Last Colonoscopy < 12 months: The date the second Colonoscopy is scheduled ~ CRC Screening Program ~ 4. Positive FOBT (PF) ~ CRC Screening Program ~ 5. Other Screening (OS) In-Patient: Date of Admission Out-Patient: Earlier of the Date the Referral was received by the: ● Facility ● Specialist (Endoscopist/Surgeons)
