ACADEMIC YEAR 2014-2015
This form is a request to enter our e-mail distribution list for updates on transportation of your student to BASIS
Phoenix Central. This is not an obligation to purchase.
By filling out this form, you are giving us your student’s information, your information and the major cross streets
where you live or your address, so that we can design routes for the transportation of as many students to BASIS
Phoenix Central as feasibly possible. Not all areas will be covered. A minimum of 15 students travelling round trip
is required for any route from a specific area to materialize for Academic Year 2014-2015. There is an annual
transportation service agreement requirement for any route to any new school.
By filling out this form, you will enter into an e-mail distribution list so that we can keep you updated as to the
status of the transportation routes. If, at any point in time, you wish to be removed from our e-mail distribution
list, you can notify us immediately and you will be removed.
