DIRECTIONS (WITH SCREENSHOTS) FOR BIOMETRIC SCREENING REGISTRATION


Go to https://my.questforhealth.com/login
Click the green Register Now link in the green box under ‘Sign Up Now’

Enter the registration key CC2016 in the box under ‘Enter Registration Key’
Click the green Continue link

Enter your first name, last name, and date of birth in the green boxes indicated
o If you are in the Cigna health plan please enter your first and last names as they
appear on your Cigna card
o If you are not in the Cigna health plan, enter your legal first and last names
Using the drop-down box under ‘Relationship’ choose
o Employee if you are a benefits-eligible CC employee
o Non-Employee if you are the spouse or domestic partner of a benefits-eligible employee
o Non-Employee if you are a CC Retiree or the spouse/domestic partner of a CC Retiree
Create a Username and Password (SAVE THESE IN A PLACE YOU CAN FIND THEM!!!)
Click the I Accept button under ‘Participant Consent’
Click the I accept button under ‘Terms and Conditions’
Click the green Continue button

Choose your gender from the drop-down box
Enter your phone number (including area code)
Enter your email address (this is where you will receive notification that your results are ready!)
Enter your mailing address (this is where you will receive a hard copy of your results!)
Click the green Register button

Wait a few seconds while the system redirects you

Answer all of the health questions (note that “I prefer not to answer” is an option if you do not
want to answer any of the questions)
Click the green Finish button at the bottom

Receive notice that your registration is complete
Wait a few seconds as the system redirects you into the scheduling module

Click on the What is a Wellness Screening link for information on the screening
Click on the Why Should I Participate link to see the incentives available for participating
Click on the green Get Started button to move to the next step

Click on the green Participate Now button to schedule

Click on the At an Event box to schedule your biometric screening onsite in the Bemis Great Hall
on 10/27/16
Click on the At a Patient Service Center box to schedule your biometric screening at a Quest lab
any day between 9/23/16 and 11/10/16 (you can choose the Quest lab from among those
listed; click on ‘more information’ beside each lab listed for their hours of operation)

If participating onsite at CC, October 27 will already be marked on the calendar
Choose the time you prefer from among those available

You do not have to choose the basic biometric screening … that is assumed if you are
registering. CC covers the cost of the basic biometric screening for all benefits-eligible
employees and their spouses/domestic partners!
If you would like to take one of the additional tests
o you can see more information about the test using the More Info link
o Click the radio button beside each additional test you want
Please note that while the site indicates these additional tests are free,
they are not. Costs are:
o Complete Blood Count – $18.00
o A1C -- $14.00
o Liver – $41.00
o Vitamin D – $30.00
o Prostate (men 40 and older only) -- $12.00
We can accept cash/check for payment of extra tests the day of event – no credit cards!!
After you have made any (or no) selections, choose Next

YOU ARE NOT REGISTERED UNTIL YOU CLICK CONFIRM!
Remember to fast for 9-12 hours prior to your appointment time … nothing to
eat or drink EXCEPT water and any prescribed medications. If you eat or drink
anything during the fasting period (including coffee!) you will not be able
participate!

If participating at a Quest lab, choose the lab you prefer – when you click on it, that lab will be
highlighted blue
You can click on the green “More Info” link under each lab listed to see information on hours of
operation

11/10/16)

Hint: if you can’t find a time that works for you, click the green link for Try another location to see if
another Quest lab has the date/time you want.
