place to live active, safe and healthy lives…

Health and Social Care services must change to meet the needs of people
across Scotland. Locally our Health and Social Care Services will soon become
one organisation.
Why Are Services Having To Change?
People are living longer and often have very complex health and social care
needs. This increase in demand for services means we must look at different
ways of working with people to ensure they have choice and control
and can live safely and independently in their own homes for
longer. It’s important people get the right services at the
right time and money is spent on what people need.
What Will Be Different?
Locally our GPs, Hospitals, Health Workers and Social
Care staff, along with other organisations, will
work side by side to share information and take a
much more co-ordinated approach with a focus
on identifying what really matters to people and
working with them to achieve this.
What Will That Look Like?
In order to meet the changing needs of society,
services will work in partnership with individuals
and communities to make best use of, and be
creative with, what we have available locally to meet
these needs.
• Listening to and understanding what you need to improve your health, wellbeing and quality of life and working with you to help you achieve this.
• Working with you to ensure you have the information and access to any
support you may need to help you better manage your health condition or
disability.
• Ensuring your experiences are positive and services that are provided are
personalised and safe.
• You will be an equal partner in your health and social care planning; and will
be supported to take more responsibility to keep well.
• We will identify and support people who look after others and help them
continue their caring role.
