MANDATORY WAIVER, RELEASE AND INDEMNITY AGREEMENT
For and in consideration of 3-Step Kicking and any and all sponsors, coaches, volunteers,
affiliates and other individuals or groups associated with 3-Step Kicking including
owners of the facility or park districts, or school districts the camp is held at, its officers,
employees and agents permitting Participant to participate in the selected Kicking Camp,
Private or Group Training. Participant, by agreeing to these terms, hereby voluntarily
indemnifies, releases from liability and holds harmless all Camp Sponsors for any
accident, injury, illness, death, loss, damage to person or property, or other consequences
suffered by Participant or any other person arising or resulting directly or indirectly from
Participant's participation in 3-Step Kicking Camps, Private or Group Training. In the
event that Participant is injured, Participant agrees to assume any financial obligation,
either through Participant's personal health insurance, or through some other means, for
any medical costs which Participant incurs. Camp Sponsors assume no responsibility for
any medical expenses, injury, or damage suffered by Participant in connection with
Participant's participation in 3-Step Kicking Camps, Private or Group Training.
Participant further agrees to conform to all rules and regulations adopted by Camp
Sponsors relating to 3-Step Kicking Camps, Private or Group Training.
IT IS THE INTENTION OF PARTICIPANT BY AGREEING TO THESE TERMS TO
EXPRESSLY ASSUME ALL RISK OF PERSONAL INJURY, DEATH, OR
PROPERTY DAMAGE UPON HIM/HERSELF, TO THE EXCLUSION OF CAMP
SPONSORS, AND TO EXEMPT AND RELIEVE ALL CAMP SPONSORS FROM
LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL
DEATH.
Participant further agrees that Participant, his/her parents, assignees, heirs, guardians, and
legal representatives will not make any claim against, sue or attach the property of Camp
Sponsors for any loss or damage resulting from Participant's participation in 3-Step
Kicking Camps, Private of Group Training.
PARTICIPANT IS AWARE OF THE POTENTIAL DANGERS INCIDENTAL TO
ENGAGING IN THIS ACTIVITY, THAT THIS IS A RELEASE OF LIABILITY, A
WAIVER OF THE PARTICIPANT'S LEGAL RIGHT TO COLLECT DAMAGES IN
THE EVENT OF INJURY, DEATH OR PROPERTY DAMAGE AND A CONTRACT
BETWEEN PARTICIPANT AND CAMP SPONSORS, AND PARTICIPANT SIGNS
IT OF HIS/HER OWN FREE WILL.
