The Men’s Association Club League Membership dues cover your annual GHIN Handicap dues, placement prizes,
skins competition and weekly hole prizes. Cash or Checks (payable to CCMGA) must be either sent to:

The CCMGA has 3 specific rules that you need to be aware of for League play and abide by.
1). You must play with another league member on Wednesday League days in order to be eligible for
skins/placements.
2). Ready Golf MUST be adhered to. You are required to play within ½ hole of the group in front of your group.
Failure to do so could result in disqualification for that days play.
3). All USGA rules will apply, and we do reserve the right to audit any score, and prize money’s.
New Members without a GHIN number or HDCP Index will be contacted by the HDCP Chairman to assign a
temporary HDCP.
By paying my dues and signing this application, I agree to follow the rules and by laws of the Coyote Creek Men’s
Golf Association. I agree to uphold the highest traditions of golf and follow the USGA Rules of Golf.
