Job Title:
JOB DESCRIPTION
Wellbeing Facilitator (fixed term until 31st March 2013)
NJC Scale Point:
NJC Pt 22 – 26 (£19,621-£22,221 pro rata)
Hours:
18.75 per week and such additional hours as are required
by the business from time to time. (You will be required to
work these hours during evenings and weekends)
Annual Leave:
26 days plus statutory and bank holidays (1 additional day
after each full year of service, up to a maximum of 30 days)

Location:
The Orchard Medical Centre, Kingswood, South
Gloucestershire (or any other location your employer may
reasonably specify)
Pension:
Contributory pension scheme (employers’ contribution 7% to
a minimum 3% contribution from employee).

Personalisation Services Team Leader
Principle Purpose of the Job
To work in partnership with The Orchard Medical Centre (TOMC) to provide increased
access to support services to meet the needs of Primary Care users with psychosocial
and economic needs. To facilitate and support links between primary care and the
voluntary community sector (VCS), increasing their capacity to work together. To
support patients to access appropriate VCS services and provide follow up and
practical support where necessary. To enable primary care staff to link patients to
appropriate support in the community.
Key Duties and Responsibilities
Operational Duties
1. To carry a caseload of clients and develop and implement effective support
packages that will address the specific needs of individuals, primarily through
brokerage into relevant support services, and draw on a multi-disciplinary
approach.

2. To complete needs assessments and risk assessments with individual service
users.
3. To create and agree an action plan with individual service users and provide the
necessary support as identified in the plan. Assist and encourage service users to
make their own decisions and choices.
4. Conduct reviews of support plans with service users and participating
services/agencies.
5. To liaise with, develop and maintain good relations with TOMC and Partners.
6. To assist with community resource mapping exercises and maintain a database of
community resources.
7. To assist with patient needs mapping to inform service development in relation to
localised needs.
8. To keep case files and database up to date and be able to provide monitoring
information as requested by the Personalisation Services Team Leader, DHI and
TOMC.
9. Assist the Personalisation Services Team Leader to evaluate and develop the
service.
10. Positively promote the service to external agencies and relevant statutory bodies.
11. To maintain records in line with all operational procedures.
12. To work closely and establish effective working relationships with a range of
agencies to facilitate a ‘joined up’ approach to action plans.
13. To broker and establish new partnerships between public and voluntary sector
agencies to enhance service delivery and access to services.
14. To liaise with relevant agencies and, with the agreement of the service user, act as
an advocate with those agencies on the service users behalf.
15. To develop and sustain professional relationships with service users, partner
agencies and appropriate external agencies.
Organisational Responsibilities
1. To perform as part of the DHI team, attending and participating in regular team
meetings and to attend appropriate training courses.
2. To work in accordance with the DHI Lone Working policy and procedures.

4. Participate in the planning processes and contribute to the establishment of DHI’s
business plan.
5. To work with the Personalisation Services Team Leader in the periodic review of
service policies and procedures.
6. At all times adhere to relevant legislation, good practice and policies and
procedures, including Child Protection, Safeguarding Adults, Health and Safety,
Confidentiality and Equality and Diversity.
Monitoring and Administration
1. Maintain accurate records and produce written reports of work as required.
2. To carry out all necessary administration in relation to casework task including
monitoring

Person Specification
Essential:
Demonstrable experience of working with people with health and social needs, or
community based work with socially excluded groups.
Experience of community needs and/or resource mapping.
Demonstrable experience of partnership working.
Ability to understand diverse and complex client needs and to link these needs with
quality initiatives.
Experience of managing a client caseload, assessment process and support planning.
IT literate with working knowledge of Word, Excel and databases.
Excellent verbal communication skills and report writing skills.
Knowledge of Motivational Interviewing and Brief Solution focussed working.
