Completing the Online Approval Request for Use of Alcoholic Beverages 1. Navigate to www.virginia.edu/vpsa/alcohol_request_intro.php. 2. To start the form, input the name of your event and the date of the event. Note that it is important to choose a descriptive name for your event. Because we use this information to identify your event, choosing an elaborate name (e.g. Retirement Dinner for Angela M. Davis) over a simple name (e.g. Dinner Party) will help distinguish it from others. Please do not include special characters (e.g. @!#$%’) in your event name. Click “Continue.” 3. If this is an event that you have submitted but need to make changes to, be sure to check the checkbox to the right. This will pull up the existing form. You can use this feature to view a previously submitted event or make changes and resubmit if your initial request is denied. If this is a new event, you do not need to check this box. Click “Continue.” 4. The full form will appear. Be sure to read all instructions carefully while completing the form. Required fields are marked with an asterisk (*). Note that data cannot be saved and refreshing the page will cause you to lose all data entered into the form. The page does not timeout. Be sure to input answers in the appropriate format (e.g. phone number, email address). Completing the Online Approval Request for Use of Alcoholic Beverages If you expect to have persons under 21 attending your event, you will have additional questions to complete on the form. Be sure to read U.Va.'s policy on the Use of Alcoholic Beverages and Prohibition of Other Drugs and Guidelines for Hosting a Safe Event. 5. In completing question #3, “Type of alcohol served & total quantity provided,” be sure to include the total number and what type of container in your response (e.g. 48 12-­‐oz. bottles). If you do not know the exact quantity, we require that you list the type of container and then attest that on average, the total amount of alcohol served will not exceed more than one drink per person per hour. 6. Before submitting the form, be sure that you have selected the appropriate School, if applicable. If you are submitting an event for the Darden School of Business, the U.Va. School of Law, or the School of Engineering and Applied Science, you will need to choose the appropriate option. If you are not with these three schools, you do not need to select an option. Image for step #6 7. Be sure to initial and date the form. If you have supplemental documents to submit with your form, please email them directly to Ms. Julie Caruccio at [email protected]. Click “Submit Event.” 8. You will receive an email confirmation to the email address that you supplied in filling out the form. You will be contacted within (3) business days. If you have not been contacted by then, please call Jessica Strang at 434-­‐924-­‐0901. If you receive notification that your request has been approved, please forward to the appropriate persons (e.g. if it is required by your space contract).
