or a “Super Ticket” for
$750.00 (fee for one/four person team, two mulligans/person, entry fee for skins, one door prize
ticket/person, and a hole sponsorship)

Tournament format will be captain and crew. Fees cover golf for all four players, carts, light lunch, dinner and
prizes. Before the tournament the putting green is available as well as a free bucket of balls for the driving
range. Send team fee with the registration form or go online to pay. Field is limited to 36 teams.
