Please fax or mail your completed application form in advance of the April 12 deadline. You may also register online at
www.usef.org by signing in to your ‘USEF Login’.
We will email or fax a confirmation of receipt of your completed application. Online registrations receive immediate
confirmation on the web page, no additional email will be sent.
Attendance sheets must be signed at each session to receive credit for the clinic. If you arrive more than one hour later
than the scheduled starting time, or leave more than one hour earlier than the scheduled adjournment, you will not
receive credit for attending the clinic.
The measurement refresher session will be available as an online presentation only. Information will be emailed once
it is available.

DIRECTIONS
Bluegrass Airport to Hotel- exit airport left onto Man O’ War Blvd, turn right on Versailles Rd, take ramp onto New Circle Rd
North, take Exit 9B for KY-922 N/Newtown Pike towards interstate, turn left and take a right on Stanton Way, hotel is at the
end of the road on the left.
From North (Cincinnati) to Hotel - take I-75 South to exit 115 Newtown Pike KY-922, turn left on Newtown PK, turn left and
take a right on Stanton Way, hotel is at the end of the road on the left.
From West (Louisville) to Hotel - take I-64 East to exit 115 Newtown Pike KY-922, turn left on Newtown PK, turn left and take a
right on Stanton Way, hotel is at the end of the road on the left.
From Fairfield Inn to USEF - turn right on to Newtown Pike, go 2 miles and turn left on Iron Works Pike (4-way stop), use KY
Horse Park main entrance (past the campground entrance). Bear right out of the roundabout and proceed until the road turns
left. USEF is the second building on the right. Please park in the parking area directly across from building.
