Agreement for Receiving Course Lecture Materials
Under Section 504, Subpart E Postsecondary Education, of the 1973 Rehabilitation Act and
the Americans with Disabilities Act, institutions of higher education must provide
reasonable accommodations to a student's known disability and may not deny equal access
to the institution's programs, courses, and activities. Obtaining a copy of the course lecture
materials, including copies of overheads and multimedia presentations, is a reasonable
accommodation for students whose dosumentation calls for this accommodation.
The Faculty/DSO have right to require a student who receives course lecture materials
(including copies of overheads/multimedia presentations) to sign and present the form to
the instructor.
As a qualified student with a known disability, I have been granted the reasonable
accommodation to receive lecture materials (including copies of overheads and multimedia
presentations) for courses in which I am enrolled. Due to academic integrity issues and the
possibility that some of the materials may be under copyright, I agree to abide by the terms
of this Agreement as a condition for receiving the course lecture materials.

anyone to see or copy (including making a backup copy of electronic materials) the
instructor's lecture notes overheads, or multimedia presentations. Additionally, I agree that
neither will I profit financially, nor will I allow others to benefit personally and/or financially by
using the course lecture materials I receive in
(Course Number, Course Title, Instructor)
At the conclusion of the semester, I agree to return the course lecture materials to the faculty
member. I understand that a violation of this Agreement may subject me to discipline under
the Code of Student Conduct or subject me to liability under copyright laws.
