CSSC Registration is Now Open for Fall 2016
Fall Players need to register by June 15
Dear Parents,
1. The CSSC Fall 2016 Registration system is now open at
https://www.youthleaguesusa.com/epys/16-17/0051/014/Welcome.html
The Registration Fee is $145 for the 2016-17 soccer year which runs August 1, 2016 thru July 31, 2017.
Uniform costs are in addition to that.

If you are unable to pay your full balance when you register your player, there is a payment plan
available. You will be able to select your payment option under the "Team/Payment Selection" section
of the registration program.

If your player is new to CSSC, before June 15, CSSC will need:


Photo of your player (school photos are fine.)
Copy of your player's birth certificate or passport
You can give/email these items to your team registrar or your coach.
2. When you go to the registration program, you will first need to select the status of your player-- New to
CSSC or Returning to CSSC. If you are good with computers, you should be able to run thru this without
reading these directions. When the actual registration form opens, you will see a long page with several
sections:

2. Welcome Page
3. Registrant Information asks for basic information about the player.


If the player is a Returning Player, some of this information will already be completed.
Remember:The yellow highlighted fields are required fields.

4. Team/Payment Section asks about the team/teams you will be playing on and your payment choice.

Choose how many teams you will be playing on with CSSC. If you are playing with another club
and CSSC, just choose 1 team. You will enter Primary Team info even if you are primary with
another club! We will make this change when completing your registration. If you are playing
on 2 CSSC teams, you will choose your Primary and Secondary Team Names.

Under Select your Payment Option - If you choose Option #3 payment plan, be sure to read the
blue box instructions, check the box and insert your full name.

5. Family Information


General information on parents/guardian is collected here.
Parents also have an opportunity to volunteer to help the club.

6. Emergency Information





Please give us the name and phone number for someone to call if a parent is not available and
your player has an emergency situation at a practice, game or tournament.
Also let us know any medical conditions or problems your player has.
Click on " Medical Release Form." A pop-up message will come up. Click "OK".
Print out the Medical Release Form.
Later, fill it out by hand be sure to give it to your player's coach.
7. Concussion Information



Answer both boxes.
Read and check the check box regarding your electric signature
Enter your first/last name.

8. Communication Information


An email address is needed so the club and the coach can communicate with you.
A text messaging address for you also can save a coach several phone calls to let you know practice is
cancelled or a game has been moved ot another field or date/time.


This section is self-explanatory

10. CSSC College Scholarship



Starting with the Class of 2013, CSSC will give a $250 college scholarship to a Danville girl
graduate and to a Danville boy graduate who have played soccer for at least 3 years.
Scholarship winners do not need to have played high school soccer and do not need to play
college soccer.
You may donate to the CSSC College Scholarship fund here if you wish.
11. Important Policy Information


Read each section carefully and then check the box to the left of that section.
Policies here are:
1. The Eastern Pennsylvania Youth Soccer (EPYSA) liability release. CSSC players register thru
EPYSA .
2. The CSSC Code of Conduct
3. The CSSC Snack Shack parent commitment. These will be assigned by your team's Auxiliary
Rep.

12. Submit Registration Information


After the 3 Important Policy Information boxes are checked, click " Submit Registration Information".
A box will come up asking you to confirm your player's date of birth and gender.
 Check to make sure they are correct.

Confirm by clicking "OK"

13. Summary Page
 If you have another family member to register, click "Register Another Player"
and you will be able to repeat the process you just went thru with your first player.
o When you are all done registering your last family member, all members will be
listed on the Summary Page with the Total Due.
 On this Summary Page, you can see the total
amount due:
 click "Proceed to Secure Payment Options"

14. SECURE PAYMENT OPTIONS
 Click, CHOOSE and you have these two options. Click one of them.

15. Payment Transaction page



If you previously chose “Option 3 - payment plan”, your amount charged will show as $90 on this
page. Your second installment payment (50% of balance) will be taken from your credit card in 4
weeks, your final payment (remaining 50% of balance) will be taken 8 weeks from the day you
registered.
Fill out the payment information for your credit card or electronic check
Click "Submit Payment"
