This letter refers to all members of this household registered with the Trinity Healthcare Centre, Lawley Medical Practice and Oakengates Medical Practice Dear Patient/s Merger of the Trinity Healthcare Centre, Lawley Medical Practice and Oakengates Medical Practice on 1st April 2017 Trinity Healthcare, Lawley Medical Practice and Oakengates Medical Practice have applied to merge their practices to create one single practice; their application has been approved by Telford and Wrekin CCG. Trinity Healthcare, Lawley Medical Practice and Oakengates Medical Practice will therefore formally merge on 1st April 2017 and the merged practices will be known as Teldoc. Teldoc will currently retain all of its existing premises, i.e. Lawley Medical Practice, Oakengates Medical Practice, Aqueduct Surgery, Lightmoor Surgery, Malinslee Surgery, Leegomery Surgery, Hadley Health Centre, Madeley Medical Practice and Hadley Highfield Clinic. All patients will therefore be able to continue to access these surgeries. On merger all of the existing practice telephone numbers will remain the same as you currently use. The GP Partners at Teldoc will be, Dr S Allen, Dr C Effiom, Dr O Gabinet, Dr M Haque, Dr M Qureshi, Dr D Karthikeyan, Dr Narasegowda, Dr Shenoy, Dr A Pringle, Dr C Engelberts, Dr Bhachu Dr Nolan, Dr L Shui, Dr T Williams, Dr T Hughes, Dr J Darcy, Dr I Chan. The wider practice teams will also remain the same. The merger will aim to deliver the following benefits to patients – A larger team working together to maintain and improve the quality of care. The sharing of skills and expertise of staff from the three former practices. Development of an improved appointment system that will, over time, offer many ways to contact and access the service. More co-ordinated care that draws on skills from both practices to develop teams to support patients with long term and complex conditions. A wider range of services and more choice for patients. As an existing patient at Trinity Healthcare, Lawley Medical Practice and Oakengates Medical Practice, you can access full medical services at any practice site when the health record systems merge in July 2017 (Teldoc will inform patients when the systems merge are taking place), up until that time please continue to use your previous GP practice.
