Who we are
Aims of the service
The Children’s Hospital @ Home Team
service consists of a team of trained
children’s nurses with a wide range of
experience in caring for sick children.
The H@H Team will visit within an
agreed 2 hour time-slot in accordance
with your child's treatment needs and
your doctors request.
We provide acute, home based nursing
care and support to children, young
people and their families who have had
an acute episode of illness and are
considered suitable by their GP or
Consultant to continue their care at
home with our support.
Both your child and family will be
treated with dignity and respect.
You will be included and consulted in
all decisions regarding your child's
care.
We recognize that we are guests in
your family home and staff will
conduct themselves in a professional
manner at all times.
You will be given a direct contact
number to Hospital @ Home to discuss
any concerns during working hours.

What we expect
That you ensure that either the child’s
parent or guardian is present when we
visit.
That you will work in partnership with the
service in the best interests of your child.
That you will behave in a polite and
respectful manner.

If you need advice and support outside of
these hours, please contact the 111 service
by dialing your normal GP phone number
