Complete the Drug Testing and Background Check
Consent Form on-line during Program Orientation.
Pay deposit of $107.75 on-line to Castle Branch.
This deposit is non-refundable/non-transferable and is
paid directly to Castle Branch on orientation day.
Acceptable payment methods include: MasterCard, Visa,
debit card, and electronic check payment.
Complete the Change of Major Form.
Complete all Financial Aid paperwork for 2017-2018, if
you have not yet done so. Payment of tuition is required
before the start of the Fall Semester.

Registration will be completed during
Program Orientation. Please defer
all questions concerning registration
to your specific Program Coordinator
during Program Orientation. Program
Orientation Dates are provided on the
acceptance letter.
Checklist
Complete the Health Packet by August 11, 2017.
• The completed packet must be submitted to
Program Coordinator by the deadline. Your Program
Coordinator may require a different date depending on your
program and will let you know during orientation.
• Immunization records/documentation must be submitted
electronically to Castle Branch
• Students will create an account through Castle Branch during
Program Orientation.
• All health records; physicals, immunizations must be
uploaded into Compliance Trackers Medical Document
Manager, which is an electronic-online process and is
administered through Castle Branch.
• Students that do not have health forms submitted by the
deadline will be dropped from classes.

CPR:

Purchase books and suppliesincluding CPR mask and work book
prior to the start of class.

If you arrive more than 10 minutes
late you will not be admitted to
CPR and will be dropped from the
course and have to re-register.
Students are not permitted to
attend clinical without a valid CPR
card.

Drug Screen/ Background Check paperwork
to be completed during Program Orientation.
Each student will set up their individual
account with Castle Branch.

Health Records must be up-loaded to Castle
Branch by August 11, 2017. Your Program
Coordinator may give you a later date at
Program Orientation depending on your
program.

We will not waive any of the requirements;
they are set by our clinical sites.

You must provide exactly the type of
documentation we request, regardless of
what a healthcare provider might tell you
that you need. Each section of the physical
must be signed and dated by the physician.

These documents will be uploaded by
the student after an account has been
set up during Program Orientation.
Some Instructors may opt to provide
their students with the codes and have
them register with Castle Branch at
home, after the program orientation is
completed.


Tetanus – must be within the past 10 years

2-Step PPD (TB Skin test)

Step 1 needle is placed under the skin
subcutaneously via tuberculin needle. 2 -3 days later
the test is read by a health care professional.

Step 2 repeat the above procedure 1 additional time
7-10 days after Step 1 is completed. A total of two
separate needle sticks are required.

Both steps are required.

Flu Vaccination must be completed by Oct. 6, 2017

Your Instructor may give you a later date depending
on your program begins clinicals


VET Program Students ONLY need:

Physical Exam

Hepatitis B – 3 vaccines or positive
titer (Please note: any series of
vaccines must be “started” by the
deadline, not necessarily completed by
then)

Tetanus – must be within the past 10
years

(Flu vaccine is recommended)
CPR


CPR – Must be American Heart
Association BLS.
You will need a current AHA BLS
card. If you have one, we need a
copy. It has to be kept current so if
it expires before graduation, you
need to get it renewed. If you don’t
have one, you will need to take the
CPR course. You have two choices.
CPR-Option 1

Take it through Continuing
Education

Cost is $99.00 and includes book and
mask, that will be given to you when you
get to class. (recommended way)
CPR- Option 2

Register for AHS 106

See handout with CRN’s, this is “hidden”
so the only way to register is to use the
CRN’s.

Cost depends on your county but will add
anywhere from $165-$250 to tuition

Book and mask HAVE to be purchased and
brought to class or you can’t stay. Book is
approximately $20. The mask is
approximately $10.
CPR

If your card is less than 6 months old
your instructor may allow you to use
your current card if you are enrolled
in a certificate or diploma program.
Please bring a copy of your card to
Program Orientation and AHS 106
will not be required.

For Associate Degree Programs if
your card expires during the course of
the program, it will be the students
responsibility to maintain CPR
certification.

prior to Program Orientation.
Students cannot register for classes
during program orientation if the
change of major form is not submitted
in time for processing by Student

of $107.75 to Castle Branch.

Requirements

All students must complete the mandatory
On-line Health Information Science
Session in Pathway.

If you have not completed this mandatory
on-line orientation Students Records will
not process your change of major form and
you cannot register for your courses.

If you have not taken a course here at the
college for more than 2 consecutive
semesters you are not considered active.

You will need to up date your admissions
application before Student Records
processes your Change of Major form.
Requirements

Applications can be found on-line under
the admissions tab on the PTC website.
Your Change of Major Form cannot be
processed by Student Records until
admission applications are current.
Failure to do so will prevent registration.

