Sun Energy Program Application Instructions 1. Before filling any of the applications forms be sure that you have read, understood and agree with the State Energy Program Regulation. 2. FORM S-01 (Rebate Reservation Form) a. Applicant Information: Residence, business owner or business representative should fill the applicant information blanks indicating the name, address and contact information of the residence or business where the solar energy system will be installed. b. Facility Owner: Fill this area if the owner of the residence or building where the solar energy system will be installed is different from the owner of the residence or business. c. Contact Person Information: Indicate here the name and contact information of the person or representative of the Applicant or business that knows about the project and can answer technical questions about the project and application information. d. Proposed Project Address (if different from applicant address): Indicate here the project physical address if different from the address indicated in the Applicant Information section. e. Rebate, Savings and Other Incentives: i. Estimate annual energy production or savings: This is the expected energy production or savings of the system in KWH/year. For this calculation multiply the System Rated Output (w) times 0.001 (kW/W) times 365 days times the Peak Sun Hours (as determine by the installer). (System Rated Output) X (0.001) X (365) X (Peak Sun Hours) ii. System Rated Output: This is the system rated output in watts (W) base on the Standard Test Conditions (STC) wattage rating of the solar panels. iii. Total System Cost: This is be the total cost of the photovoltaic system as indicated in the State Energy Program Regulation. iv. Expected Rebate: This is the expected rebate for the solar energy system. This number shall be according to the State Energy Program Regulation. v. Other Incentive: Indicate here any other incentive (monetary or non monetary) received or expected for the construction of this project. vi. Quantity: Indicate the quantity of the other incentives. f. Required Documents Checklist: Make sure that all the documents indicated in this area are included in the application package. Check each box after you have collected each document. g. Applicant’s Signature and Date: The applicant or applicant’s representative shall sign and indicate the date in this area. h. Owner’s Signature: The owner of the building, if different from the applicant, shall sign and indicate the date in this area. 3. FORM S-02 (Installer Information Form): a. Installer Information: The installer should fill this information indicating company’s name, address and contact information. b. Proposed Project Address: The installer shall fill this area and indicate the physical address were the solar energy systems will be installed. c. Description of Work to be Performed: The installer shall describe the work to be performed for the installation of the solar energy system. d. Installer Certification and Agreement: The installer shall fill, sign and date the agreement. 4. FORM S-02.1 (Installer Information Form – Site Sketch): Installer shall draw or include a drawing indicating and describing the intended location, size, capacity, solar obstructions and connections of the new solar energy system. 5. FORM S-03 (Vendor Information Form): a. General Vendor Information: The vendor shall fill this information indicating the company’s name, address and contact information. b. Owner or Contact Information: The vendor shall fill this information indicating the contact person information regarding this specific solar energy system. c. Equipment Information: The vendor shall indicate the information of the EAA certified equipments to be installed for the solar energy system. d. Equipment Warranties: The vendor shall indicate the equipment warranties and include supporting documentation. e. Vendor Agreement: The vendor shall fill, sign and date the agreement.
