Please complete fully and return to a league meeting at the Queens
Head on or before Thursday 3rd August 2017. Forms must be
returned fully completed with all player and team registration fees.

Please complete your player details on the page overleaf. Please note
that there is a registration fee of £20.00 per team* plus £2.00 per
player. (* Free registration for teams that attend the previous
season’s Annual General Meeting).
Five further payments of £25.00 per month are then payable at each
league meeting for the first 5 months of the season. Failure to pay
these fee’s can result in expulsion from the league.
Each team is required to be represented at each league meeting to
hand in scorecards and pay fees.
You should have a minimum of 9 players per team. Additional players
can be added to your team up to the mid point of the season for a
£2.00 fee per player.
