Entry fee $5 court hire/balls payable in cash on the day
This form must be completed and returned to the south coast convener no later than Thursday
9 February 2017.
The convener will then email you the consent form to be used along with the South Coast Code of
Behaviour that must be adhered to by anyone attending the trial. Consent forms must be fully
signed and returned to the convener either prior to the trial or handed to them on the day.
Anyone attending the trial without a signed consent form will not be permitted to trial.
School sports uniform to be worn –representative gear not permitted.
Note:
1. 12 boys may be selected to attend the CHS Individual Championships at
Parramatta from 28 February to 3 March 2017 (days of competition depending
on age division).
Cost for CHS Individual Championships entry is $40 payable to “NSW CHS”.
2. The South Coast Championships will be held at ACE Tennis Complex (and
Lysaghts tennis courts if required). Students will be expected to travel and play
at each venue if required.
3. The convener has complete control of the championships and may vary any
condition.
4. Students who wish to be selected in the South Coast Regional team must
compete at the opens trial on Tuesday 14 February. Players attending the
trials must be available for the CHS Championships and Basford Shield state
titles. The NSW CHS Boys Tennis Championships will be held in Bathurst from
Tuesday 22 to Thursday 24 August 2017. Note that this is now an opens
event.
5. Students must travel to and from the venue/s via their own transport.
6. Competitive tennis players only should attend trials (see form following).

Please nominate only those players who are of a high standard and available to participate in the
CHS championships and state carnival.
Opens boys
There is a limit of 3 students per school and the numbers will be capped at 30 for the trials. If
you have any more interested students, please run an in-school trial as only the top 3 mentioned
students will be considered.
