TRC Community Health Center is recognized by the
National Committee for Quality Assurance (NCQA) as
a Level-III Patient Centered Medical Home (PCMH).
A PCMH is a health care setting that emphasizes
partnerships between individual patients and their

Services
Preventive health services are offered to every patient
and each is encouraged to complete evidence-proven
preventative screenings. This helps to promote and
protect good health. To assist patients in working towards
a healthier lifestyle, TRC Community Health Center offers
education in many forms including small classes, individual
instruction, printed materials and online information.
TRC Community Health Center tracks patient’s progress
and provides follow-up care for individuals that require
additional support. Referrals are made to the care
coordination team to help individuals secure services
such as: diabetic education, smoking cessation, and weight
management. Our team can also assist with transportation
and help manage these important pieces of care. TRC
Community Health Center strives for a team approach
when supporting our patient’s wellness goals.

Community Health Center physicians can provide
health services to your entire family.
The goal of our providers is to prevent and treat
illness, protect your health and allow you to live as
healthy as possible.
Our physicians are board-certified to ensure optimal
care for our patients and their families. Our physician
assistants also specialize in family medicine and see
patients of all ages.
Follow My Health
TRC Community Health Center has adopted a program
called Follow My Health. This is an online patient
portal that enables patients to take a proactive role in
managing their care at their own convenience. Please
