1. Please read the Round Two Quality Pre-accredited Delivery Network Expression of Interest
Guidelines March 2010 before completing this application form.
2. This application form is designed to be completed electronically. If you do not have
electronic signatures you will need to print and sign the Declaration/s (Part C). This applies
to the lead agency (EOI applicant) and network partners. These must be scanned and
emailed or faxed for inclusion with the application by the closing date and time.
3. The closing date for enquiries is 5pm Friday 9 April 2010
4. The closing date for Expressions of Interest is 2pm Friday 16 April 2010
5. To submit an Expression of Interest you must, by the closing date and time:


Please note that Expressions of Interest received after the closing date and time and
incomplete submissions will not be considered for funding.
