Six Monthly Progress Report





Mind Australia’s Research and Evaluation Committee’s (Committee) approval for the project is built on the
expectation that each six month, from the date that the support is given, a progress report is submitted by the
Principal Researcher (or delegate) to the Committee.
All sections (A, B and C) and all applicable questions are expected to be completed.
Upon completion of the project, a final report (separate template) must also be completed and submitted.

The Committee would appreciate receiving permission to post any of the study’s newsletters, findings and
publications on the Mind website (please see Sections B (questions 2 and 3):
