What are my points worth?
For each activity completed during the year, you earn points. These points accumulate to a maximum of 200
points. Your points are redeemable for one dollar per point in the form of a Gift Card selected from Amazon’s
gift card gallery. Points may be cashed in using increments of $20 and/or $25. There are several
combinations of dollar amounts you can use, and you don’t have to cash in all your points at one time.
For example, you have 80 points available. You could elect to have any of the combinations below, not to
exceed a total of $80:





4 $20 gift cards,
1 $20 gift card, and $25 gift card,
1 $50 gift card,
1 $80 gift card,

How do I cash in my points?
1. First review Amazon’s gift card gallery at this web link, Amazon's gift card gallery.
2. Choose from the huge gift card selection. Record the name and dollar amount for each desired card.
3. Send an email to [email protected]. In your message, include the following
information.
a. Your Full Name
b. The Member Number from your Health Benefits ID Card
c. Your date of birth
d. The Store name for the card or cards you want, with the dollar amounts.
4. The Rewards Coordinator will validate your point earnings, order your cards, and have them shipped to
your home address. You will receive a confirmation email when your rewards transaction is validated,
and ordered. Estimated delivery will be provided when possible, but expect up to 14 days for shipping.
It has been more than 14 days since I received my confirmation email and I still do not
have my cards. What do I do?

information. You will receive a response within 48 hours.
