L.E.A.F. Award Application
LEAF (Lennox Educational Advancement Foundation) is a non-profit educational foundation
that since 2006 has provided Student Awards to graduating Lennox high school students
who have demonstrated unrelenting drive, motivation, and work ethic in the pursuit of
higher education.
This application is open to seniors at Lennox Mathematics, Science & Technology Academy
(LMSTA) who are in good standing and will be enrolled in a post-secondary education
program in the Fall 2014. Criteria for selecting award candidates will include academics,
financial need, and leadership achievements. Award recipients will be notified in May 2014.
Deadline to submit the complete application is April 28, 2014.
DIRECTIONS TO APPLY: Please complete the entire application below. If there is a question that is not applicable
to you, simply write "NA". Application is due MONDAY, APRIL 28th. Please note that two (2) letters of
recommendation must be turned in by the deadline to Ms. Jimenez. One letter of recommendation must be from a
Lennox Academy Teacher, and another letter from someone of your choice. Applicants who fail to provide
supporting documents by the deadline will be considered to have an incomplete application. ONLY COMPLETE
APPLICATIONS WILL BE CONSIDERED.
