FIRST 5 MODOC MICRO-GRANT PROPOSAL GUIDELINES
REQUESTS FOR FUNDING UP TO $1,000
This application may be submitted to FIRST 5 Modoc at any time throughout the year on or before the
1st day of each month, applicants should expect approximately a 60 day turn around. Applications
will be considered at bi-monthly Commission meetings normally on the 2nd Wednesday of the month.
Having representation at the meeting your application is being considered is very helpful. Awards will
be made until the annual budgeted funds are committed and then newly submitted proposals will be
held for consideration for the following fiscal year.
Funds may only be used to benefit children ages 0-5 and their families, in the areas of Improved Child
Development, Improved Family Functioning, Improved Child Health and Improved Systems of Care.
Funds may not be used to supplant any existing funds – requests should be for new
activities/resources or expansion of existing activities/resources.
When submitting a proposal, please include 1 original and 10 copies (11 total) of the following
2 items, which are both included in this packet. Please see item 3 for further submission
guidelines:

A completed W-9 Form will be requested from all successful applicants. Applicants previously
funded under the First 5 Modoc Micro Grant are eligible to apply again if they remain in good
standing.

This narrative section should not exceed 2 pages (8.5” x 11.5”). Applicants are encouraged to be brief, but to
answer each question. If you are using a word processor, please use a font size of at least 12 points.

Staff Micro Grant Application Evaluation Guide
Name of Proposer:
A. Proposal Requirement Check
After proposal submission, First 5 Modoc staff will evaluate each proposal for responsiveness to application
guidelines and requirements. Each application will be subjected to a review to determine proposals submitted
contain all of the paperwork items needed for Commission consideration. Proposals found to be unresponsive
for any reason will be rejected from further consideration within 7 working days of submission to the First 5
Modoc office. Applications are due to the First 5 Modoc office by the 1st of each month of a regularly
scheduled Commission meeting in order to be considered during that meeting. Incomplete applications may
cause a delay in Commission consideration until the next regular Commission meeting.
B. Application Evaluation
Staff will subject each application to an evaluation process to insure all documentation is included with their
proposal. A determination will be made based upon the following criteria:
1. Was proposal completed using the First 5 Modoc Micro Grant Cover Sheet and Detail Sheet? Yes/No
2. Was the proposal submitted in a timely manner? Yes/No
3. Is the organization a non-profit with a 501 c (3) status or have they been sponsored by a 501 c (3)
organization? Yes/No
4. If they are an agency other than a non-profit is the organization a public agency? Yes/No
5. Does the proposal target one or more of the current focus areas in the Strategic Plan? Yes/No

Commission Evaluation Criteria
FIRST 5 Modoc will use the following evaluation criteria when it makes funding decisions on Micro Grants:
1. Does the project have a clear link to the current Priorities in the Strategic Plan?
(Improved Child Development, Improved Family Functioning, Improved Child Health and Improved Systems of Care)
2. Are the planned activities achievable within the timeline and the budget?
3. Does there appear to be a need for these activities in Modoc County?
4. How many young children will be positively impacted by this project?
5. Will the grant money be used solely to benefit children ages 0-5 and their families? Has the
applicant demonstrated an effort to access other resources toward this effort?
