Meaning they take the amount out of your paycheck first, then they tax you on the remaining. The tax is approximately 20%. Babysitting Max amount you can flex: • • • • You will receive a flex card and pin in the mail for the amount you flexed. $2500 – Single $5000 – Married filing jointly • The card can only be used in the US. CuraLinc • CuraLinc in a FREE employee/immediate family assistance program. • • You or your immediate family are eligible for up to six (6) free visits They will use someone that is a provider in case you would need more than six (6) visits. Sick Leave Bank Employees interested in donating: • • Hourly – have at least 80 hours of annual leave accumulated Salary – have at least 10 days of annual leave accumulated To qualify you need: Can be used by any employee that has: 1. 2. 3. Donated for that fiscal year Exceeded their annual/vacation leave You can have max two (2) request per fiscal year Open Enrollment Only during the month of May you will be able to: • Make any changes to your health insurance (Medical, Dental, Vision, Mexico Only Plan, and/or US Coverage) • You cannot make changes to your plan at any other time, unless it’s a qualifying event. • Qualifying events are: Marriage, Divorce, Birth of a Child, Loss of Coverage, and/or Court Ordered. • Participate in the Flex Program for the 2016/17 FY • Donate to the Sick Leave Bank ANY CHANGES YOU MAKE WILL BECOME EFFECTIVE JULY 1, 2017. Automatic Coverage The following are programs available to all full-time benefit eligible employees. You do not need to fill out any forms to use them at any time during the year. 1.
