Youth Proxy Form
If you are a youth aged 16 or 17, you are entitled to a vote at the AGM. However,
because only adults (over the age of 18) may vote, youth members must assign a
proxy (someone to vote in your place). To assign a proxy please fill in the form
below, sign it, and deliver the original – i.e. not a copied version – to Ten Oaks Project
office (see address below) no later than 24 hours before the start of the AGM.
Important note: Only current voting Ten Oaks Project members may serve as
proxy voters.

Important Notice
Voting by members is allowed at this year’s AGM.
For your information, the relevant bylaw section is below.
4.4. Voting by Proxy
Voting by proxy shall be permitted at any meeting of the members. At any
meeting of the members, each member who is present and entitled to vote may
serve as a proxy on behalf of one member only. With respect to all other
matters relating to proxy voting, the board shall determine at its sole discretion
the manner and form in which the right to vote by proxy shall be exercised.
What does this mean?
This means that you can ask any other member to vote on your behalf. All you
need to do is fill out this form and make sure it gets to TOP before the AGM. Just
remember that no single person can vote twice so if you ask someone to be your
proxy then that person cannot vote for themselves as well. A good choice for a
proxy vote would be someone in a member family. Each family only gets one
vote so if there are two adults in a family asking one of them to be your proxy
vote would ensure that the family still gets to vote (as the other adult could vote
on behalf of the family).
Do not e-mail this form, it must be sent by post or delivered to the address
below no less than 24 hours before the start of the AGM.
