20th ANNUAL VETERANS AND ARMED FORCES
APPRECIATION DAY FOOTBALL GAME
VETERAN OF THE YEAR NOMINATION FORM
The Veteran of the Year award is presented annually at the Veterans and
Armed Forces Appreciation Day Football Game to recognize a veteran
residing in Los Angeles County who has compiled a record of exceptional
and exemplary service as a military service member, veteran, and citizen.
The Nomination Form must be filled out completely. You may submit
additional page(s) if necessary. Letters of recommendation are encouraged
and MUST be received with your nomination form. Nomination forms
must be received at the Department of Military and Veteran Affairs, 1816
South Figueroa Street, Los Angeles, CA 90015 addressed to the attention of
Tatiana Rosas or via e-mail to [email protected] no later than Friday,
September 23, 2016. Winner will be selected at the Selection Planning
Meeting (date yet to be determined) in early-October.
NOTE: Form must be completed (or separate pages attached). Incomplete
nomination forms will be grounds for disqualification.
