We focus on recovery and reablement - giving people the support and treatment they need to manage their illness and enabling them to lead independent and fulfilling lives both in the community and in our dedicated psychiatric hospitals. We focus on recovery to ensure your patient can return to their home as soon as possible. How it works: an example Southern Health Adult Mental Health Services How you access our services You have a patient that you believe is in need of additional support with their mental illness. You contact your local Access & Assessment Team (AAT). www.southernhealth.nhs.uk/amh How we support your patient How we communicate with you The AAT carries out a comprehensive assessment, creates a detailed care plan and begins a 12 week programme of psychological therapy. We will contact you to inform you of the results of the assessment and the care plan that we have created. After 12 weeks, your patient is transferred to their local Community Treatment Team for longer term intervention. We will tell you which CTT your patient has been registered with and how to get in touch with them. Six months later, your patient experiences a crisis. The Community Treatment Team transfers your patient to their local Acute Care Team for more intensive support. We will explain the high-intensity support we are providing and tell you how to contact the H@H team. The Hospital at Home, part of the Acute Care Team, is able to help your patient through their crisis without the need for an admission. Your patient is returned to the care of the Community Treatment Team. We will tell you which CTT your patient has been registered with and how to get in touch with them. Access & Assessment Teams Southern Health Adult Mental Health Services Access & Assessment Teams provide a single point of access, acting as the ‘front door’ for all planned and unplanned referrals. They operate 24 hours a day, 365 days per year. Access & Assessment Teams are multi-disciplinary teams including psychiatrists, clinical psychologists, nurse practitioners, social work practitioners, OT practitioners, support workers/ support time recovery workers and mental health practitioners. The core function of the Access & Assessment Team is to conduct a comprehensive health and social care assessment of your patient to the point that a care plan can be reached and follow-on treatment and support identified if needed. Services provided by the Access & Assessment Team Where a brief intervention (less than 12 weeks) is required, this will be delivered within the Access & Assessment Team itself. Mental Health Act assessments will be aligned to these teams. Access & Assessment Teams will also provide additional crisis support to people under the care of a Community Treatment Teams when required. Working with Acute Care Teams If your patient is so acutely unwell that they may need admission to hospital, they will transfer the individual to the Acute Care Team, which will treat/support the person in their own home whenever possible or arrange for admission to hospital if necessary. Community Treatment Teams are multi-disciplinary teams staffed by psychiatrists, clinical psychologists, nurse practitioners, social work practitioners, OT practitioners, carer support workers, and support workers / support time recovery workers. The core function of Community Treatment Teams is to deliver the care and treatment as determined by the individual’s assessment and care plan. Community Treatment Teams provide: • Personalised, recovery-focused care • Medical interventions and medicine management • Psychological interventions • Support for carers • Occupational therapy • Assertive outreach • Early intervention in psychosis Access to Community Treatment Teams If required, we will transfer your patient to their local CTT following assessment and initial intervention from the Access & Assessment Team. You will be notified if your patient is transferred. The core function of Hospital at Home Teams is to provide intensive treatment and support to individuals in their own homes in order to prevent unnecessary hospital admission and/or to facilitate earlier hospital discharge. Our inpatient services are staffed by psychiatrists, clinical psychologists, nurse practitioners, mental health nurses, occupational therapists, support workers and support time recovery workers. We also provide psychiatric intensive care units (PICU) within our hospitals for patients who need treatment in a safer or more restricted environment. Specialist services in Hampshire www.southernhealth.nhs.uk/amh Mother & Baby (Perinatal) Services (Melbury Lodge) 01962 825507 We provide services for mothers experiencing mental illness who have babies up to one year old. We specialise in keeping mother and baby together and offer support in the community and in dedicated inpatient facilities. Eating Disorders Services (April House) 023 8081 9000 Our Southampton-based eating disorders unit offers assessment and treatment for people aged 18 and upwards from across Hampshire with a primary diagnosis of anorexia nervosa, bulimia nervosa or atypical forms of these disorders. The clinical team includes nurses, clinical psychologists, eating disorder therapists, a dietitian and healthcare support staff. Substance Misuse Services (Southampton area only) 023 8071 7171 We provide ‘tier three’ services for people aged 18-64 who require help with their problematic drug or alcohol use. We work in partnership with other local substance misuse organisations within the Southampton City area. The clinical team includes specialist substance misuse nurses, social workers, psychiatrists and healthcare support staff. Chronic Fatigue Services (Fairways House) 023 8024 1348 We provide occupational therapy based services to those suffering from mild to moderate chronic fatigue across the whole area for those age 18 years upwards. Excellence in our services Southern Health Adult Mental Health Services ImROC Research & Development italk WRAP Southern Health was selected to pilot the national Implementing Recovery through Organisational Change (ImROC) scheme which works towards ten indicators to embed recovery into everything we do. The scheme promotes recovery through changing the nature of day to day interactions and the quality of experience of those who use our services. A comprehensive service user led education and training programme is being used to educate staff and the Trust is establishing a recovery education centre to drive the programme forward. italk is Southern Health’s IAPT (improving access to psychological therapies) service, a partnership between Solent Mind and Southern Health. italk helps people with common mental health problems including depression and anxiety. The service is open to anyone aged 16 and over who is registered with a GP in the seven localities of Eastleigh, Fareham, Gosport, Havant, New Forest, Test Valley and Winchester. Referrals can be made through GPs or by self referral. www.southernhealth.nhs.uk/amh As well as providing a wide range of clinical care, the Trust also supports ongoing research activity to improve and innovate the services we offer. We also work closely with the University of Southampton as well as other academic centres to research and develop the services and treatments we offer. The Trust currenty host the South Coast Dementias and Neurodegenerative Diseases Research Network (DeNDRoN) and the Memory Assessment Research Centre (MARC). The Wellness Recovery Action Plan (WRAP) is an exciting and effective self management tool originally used in the self management of long term conditions such as depression and MS. WRAP has been successfully used locally in secondary adult mental health services for six years and is now used throughout adult services, within physical disability, learning disability, older people and with carers. WRAP is supported by all-new government initiatives that promote personalised care.
