Faculty Out-of-Cycle Compensation Form Instructions Division of Human Resources Phone (813)974-2970 / Fax (813)974-5227 / SVC 2172 Instructions for completion of the Faculty Out-of-Cycle Compensation (FOCC) form. Please contact the Provost’s office for questions regarding unique pay requests and reasons. Tips for navigating the form: Use the “tab” key to navigate the form, it will take you to all of the fields required for completion. You do not need to add any leading zeros to the Employee ID, Position # or Combo code fields. Completing the form: 1. Enter the Employee information (Name, EMPLID, College / Department, Job Title, Position #, and GEMS DeptID). 2. Select the work Period by clicking on the drop-down arrow (9-mo, 12-mo, Summer only) 3. Select the Bargaining Unit by clicking on the drop-down arrow (In-Unit or Out-of-Unit) 4. Enter the Effective date of the pay change. 5. Click on the blue link to select the Pay Increase Reason. It will take you to the Reasons page where a detailed description for each pay change reason is listed. You will not be able to select the Pay Increase Reason from the form; these reasons will only populate by using the link. 6. After selecting the link, refer to the reason details listed and contact the Office of the Vice Provost if you need clarification. You must choose the Primary increase reason from the drop-down arrow in the first box. 7. You may also choose a Secondary Reason, if applicable, but it is not necessary. Leave this box as the default unless applicable. 8. Click on the blue link to return to the FOCC form. 9. Your Pay Increase Reason is reflected on the form. Enter the Current Base Salary and amount of the increase (to base) if applicable. 11. If the employee has a current stipend, enter the amount and end date on the line for Stipend 1. If you are changing the stipend amount, enter the amount of the increase on the same line in the next column. Notice the Red box with Asterisks’? As you hover over the red box, a box will provide you with an error and instructions to fix it. You must also enter the New Stipend end date if you are making a change to the amount. 12. 13. 14. 15. 16. Enter as many stipends as apply, there are four stipend lines available. Select the Funding availability from the department or college from the drop down arrow (yes or no). Place a check mark for the Funding Source by clicking on the box(es) that correspond to your source of Funding. Complete the One Time Bonus section with the amount, chartfield and combo code, if applicable. Print and obtain all required signatures. Instructor receiving a Permanent Increase in Responsibilities (previously on approved stipend and now permanent) effective 9/1/12. 2. Instructor extending and increasing the current Stipend. New amount is effective 9/1/2012 for $6109.00 and ends on 8/31/13. Nine month faculty promoted to chair for the first time with no increase to base pay and Stipend amount for $10,000 until 8/6/13. (Convert the 9-month base pay to 12-month and enter in the cell for Current Salary.) ***Must attach documentation showing conversion / stipend amounts. 4.
