With this honor comes a commitment that will require a great deal of hard work and sacrifice during the sports season. As a member of the athletic program, your son/ daughter will be placed in the public eye. The community, student body and other family members will observe your son’s or daughter’s every move and it is my hope that they will uphold the training rules and dignity of a Lakota Falcon Athlete. Our players will represent the entire student body and faculty of Lakota Middle School every time they compete. This representation carries into the classroom and on campus. We insist that the members of our athletic program meet the high academic and behavioral standards of Lakota Middle School. It is mandatory that athletes attend study hall sessions weekly to help balance the demands of being a successful Lakota student-athlete. Each coach will determine when study hall will take place. It is also important that our student-athletes conduct themselves in a responsible, appropriate manner while in the classroom. The next page will outline some of the expectations and standards we have for your studentathlete and the consequences for failing to meet those expectations. Whether in school, at home, or on the playing field, I will behave in a mature, responsible manner and appropriately represent myself as a person, a student, and an athlete. I understand that failing to conduct myself according to the following Lakota Code of Conduct will result in disciplinary action ranging from loss of playing time to removal from the team. Academic Requirements All student-athletes are required to meet the Federal Way School District’s academic requirements of achieving an approaching or meeting standard in every class in order to participate in middle school athletics. As a member of the Lakota Middle School Athletic Program, I understand that if I fail to approach or meet standard in all of my classes for two consecutive grade checks I will be removed from the sports so that I may focus on my academic success. Behavioral Requirements As a member of the Lakota Middle School Athletic Program, I will: 1) Create, maintain, and promote good team morals and ethics. 2) Be responsible for appropriately representing my school at all times. 3) Create, maintain, and promote good sportsmanship. 4) Respect and cooperate with teammates and coaches. 5) Put my team’s success ahead of individual glory. 6) Demonstrate and promote appropriate behavior in my classes. 7) Respect and cooperate with fellow students and teachers in the classroom. Practice/ Game Requirements 1) I am expected to be at each meeting, practice and game. If I expect to be absent, I am responsible for notifying the coach by email, phone or in person. Chronic unexcused absences may result in loss of playing time or removal from the team. I must be at school for half a day to participate that day. On and off the field, I am expected to accept the decision of any coach or staff person. 2) Managers are members of the team and I should treat them with respect by their teammates. 3) If I am injured or have an injury, I will inform one of the coaches immediately, so proper treatment or action can be taken. 4) If disciplinary action taken by the coach is deemed unfair, appeals should be directed to the coach, athletic director and then the principal, in that order. 5) I understand the consequences for athletes seen or caught using alcohol, drugs, anabolic steroids, or tobacco products are outlined in the Federal Way Schools Rights and Responsibilities booklet. 6) ALL FEES MUST BE PAID PRIOR TO THE FIRST CONTEST IN ORDER TO PARTICIPATE.
