GBMC Has Built a Better System of Care for Patients, Employers and the Community October 18, 2016 John B. Chessare MD President and CEO GBMC HealthCare System 1 The GBMC HealthCare System • Greater Baltimore Health Alliance (GBHA) – Private practicing physicians – Greater Baltimore Medical Associates (GBMA) • GBMC Medical Center • Gilchrist Hospice Care 6 2 3 1 15 GBMC Advanced Primary Care Sites 5 3 Our Vision Statement 2011-2016 In order for GBMC to maintain its status as a provider of the highest quality medical care to our community, in the context of an evolving national healthcare system, we must transform our philosophy and organizational structure, and develop a model system for delivering patientcentered care. We define patient-centered care as care that manages the patient’s health effectively and efficiently while respecting the perspective and experience of the patient and the patient’s family. Continuity of care with a focus on prevention and ease of navigation through a full array of services will be the rule. Our professional staff will be able to say with confidence that the guidance and medical care they are providing mirrors what they would want for their own family. We will create the organizational and economic infrastructure required to deliver evidence-based, patient-centered care and for holding ourselves accountable for that care. This new organization will be defined by collaboration and continuous improvement. Physicians will lead teams that will manage patient care. We are moving into the future with renewed energy and increasing insight. We look forward to building relationships with both community-based and employed physicians that will form the foundation of the Greater Baltimore Health Alliance. We welcome all those who share our vision of health care as it is transformed to meet the needs of our community and nation in the 21st century. 4 Vision Phrase: To every patient, every time, we will provide the care that we would want for our own loved ones. 5 What do we want for our own loved ones? The GBMC HealthCare Quadruple Aim 1. 2. 3. 4. It’s about a relationship of trust and accountability. – Access is critical: we open at 7AM and close at 7PM or later; we are open Saturday, Sunday and holidays. – Physicians work with nurse care managers and care coordinators to maximize health by using diseasestate registries, CRISP, and care coordination and prevention. – The opposite of fragmented- poorly coordinated care! 7 So how are we doing? 8 Better Health Breast Cancer Screening 80% GBMA All 75% 70% 70.84% 70.58% 70.78% 71.71% 72.70% 72.12% 72.23% 72.08% 72.77% 73.60% 74.37% 73.79% Num / Den 8,590 / 11,641 65% 60% 55% 50% Sep 2015 Oct Nov Dec Jan Feb Mar Apr May Jun Jul Aug 2016 9 Better Health Colon Cancer Screening 75% 70% GBMA All New Goal: 70% 67.07% 65% 64.36% 68.33% 68.54% 68.23% 68.60% 68.78% 68.74% 68.57% Num / Den 13,614 / 19,835 65.71% 65.68% 65.42% 60% 55% 50% Sep 2015 Oct Nov Dec Jan Feb Mar Apr May Jun Jul Aug 2016 10 Better Health Poor Diabetes Control- Hgb A1c 40% GBMA All 35% 30% 25% 30.11% 29.71% 29.03% 28.81% 29.57% 29.42% 31.03% 31.38% 30.68% 29.99% 29.08% 28.65% Num / Den 1,371/4,786 20% 15% 10% 5% 0% Sep Oct Nov Dec 2015 Jan Feb Mar Apr May Jun Jul Aug 2016 * Note DM2 is an inverse measure 11 Better Health Diabetic Eye Exam 45% GBMA All 40% 38.55% 35% 35.45% 30% Num / Den 1,910 / 4,786 31.76% 27.81% 25% 22.66% 20% 15% 39.91% 23.64% 25.43% 20.29% 16.58% 18.11% 10% Oct 2015 Nov Dec Jan Feb Mar Apr May Jun Jul Aug 2016 12 Better Health Obesity Screening & Follow-up 85% GBMA All 80% 75% 70% 72.42% 65% 62.91% 60% 55% 65.23% 66.29% 74.14% 75.85% 77.71% 79.66% 81.34% 82.42% Num / Den 42,892 / 52,043 68.54% 55.81% 50% 45% 40% Sep 2015 Oct Nov Dec Jan Feb Mar Apr May Jun Jul Aug 2016 13 Better Health Tobacco Screening and Intervention 100% GBMA All 95% 94.64% 94.69% 90% Num / Den 49,773/ 52,566 85% 80% 83.73% 83.90% 82.86% 82.90% 83.06% 82.22% 82.22% 83.13% 83.38% 83.47% 75% 70% Sep 2015 Oct Nov Dec Jan Feb Mar Apr May Jun Jul Aug 2016 * Note: Changed to 24 month numerator in July 2016 14 Better Health Depression Screening & Intervention 100% 90% Working with Sheppard Pratt to embed social workers and Psychiatrists in all the practices! 80% GBMA All 70% 72.29% 73.06% 72.65% 60% 62.50% 69.49% 69.97% 68.28% 67.08% 65.31% 65.55% 66.45% Num / Den 33,683/ 48,137 57.39% 50% 40% Sep 2015 Oct Nov Dec Jan Feb Mar Apr May Jun Jul Aug 2016 15 Higher is Better Better Care Overall Satisfaction Score All My Sites Better Care Higher is Better Convenience of our Office Hours Hunt Valley-Saturday All Primary Care Open Saturday Sunday Hours Open Holidays Least Waste Medicare Total Cost Per Patient Total Expenditures Per Assigned Beneficiary (Annual) $13,000 $12,653 $12,352 $12,000 $12,159 $12,035 $12,014 $11,911 $11,907 $11,654 $11,785 $11,816 Total Expenditures GBHA – 7.12% Decrease (Δ $901) ACO Cohort – 0.88% Increase (Δ $87) $11,752 $11,288 $11,283 $11,000 $10,035 $11,419 $10,050 $9,840 $10,000 $9,895 $9,825$9,774$9,825$9,863$9,830 $9,929 $10,023 $9,982 $9,000 $8,979 GBHA ACO Cohort $8,000 18 Least Waste Medicare Hospitalization Rate Hospital Discharges (Per 1,000 Person-Years) 400 386 372 355 342 350 Hospital Discharges GBHA – 27.98% Decrease (Δ 108) ACO Cohort – 11.39% Decrease (Δ 39) 354 340 337 350 342 337 330 322 322 308 303 309 300 304 297 295 314 312 301 287 274 GBHA 308 277 278 ACO Cohort 250 19 Least Waste Medicare Emergency Department Visits ED Visits (Per 1,000 Person-Years) 700 683 665 662 666 668 615 614 681 671 667 662 661 651 663 650 667 682 655 ED Visits GBHA –11.30% Decrease (Δ 75) ACO Cohort – 0.83% Increase (Δ 6) 628 619 600 616 603 594 583 585 582 588 575 GBHA ACO Cohort 550 20 With More Joy The GBMC Team takes pride in improving health and the care experience while reducing cost…it’s a joyful pursuit! 21 We are building a system of care for a better future for healthcare in our region Do not doubt that a small group of thoughtful people could change the world.
