DESCRIPTION OF THE INCIDENT
J-tube sections have been locally constructed on a Fabrication Yard (Ponticelli) located a
few kilometers from ILOGS quay were the mobilization of the Orelia (Technip Construction
vessel in charge of the setup of the J-Tube) was taking place.
J-Tube sections were transferred on 2 flat trailers from the Fabrication yard to the ILOGS
quay. As the construction vessel was not ready to reception them, they have been
temporarily stored alongside the quay on top of two foam blocks to comply with the vessel
storage scheme in the afternoon of the 16th March.
The 17th march at around 14.30 it was discovered that one of the J-tube assemblies (2T)
had overturned on the quayside.

Findings:

Procedure to transfer the J-Tube from the fabrication yard to ILOGS quay on flat
trailer on two foam blocks.

Foreman in charge of the loading of the J-Tube on flat trailer at the fabrication yard
decided to load them on one foam block as truck stanchions were too low (Good
HSE practice)
Upon arrival of the truck transporting J-Tube on the ILOGS quay side, construction
vessel could not reception them directly aboard their vessel (Tidal issue + welding
operation being performed on the side of the vessel)
Only 2 trucks were available to transfer the 12 J-Tube sections from the fabrication
yard to Ilogs quay.



It was finally decided to temporarily store the J-Tube sections on the quayside
In order to comply with the offshore installation procedure and to gain time on the
mobilization of the construction vessel. J-tubes sections were installed on top of 2
foam blocks on the quay side.

There was a deviation in the mobilisation procedure: J-tube were temporarily stored
on quayside whereas they should have been directly loaded from truck to
construction vessel
No MOC and risk assessment performed following the deviation of the procedure
Temporarily storage on 2 foam blocks along the quay was identified as not good
HSE practice nevertheless no action was taken
Foam blocks used for the storage of the J-Tube did not have all the same
characteristics (Low density and High density).
Wind gusts were observed along quayside prior the incidents





IMMEDIATE ACTIONS



All J-tube sections temporarily stored on the quayside on 2 foam blocks have been
stored on 1 foam block
Temporarily storage area was barriered off
J-Tube were then transferred to Orelia and sea fastened as per procedure

RECOMMENDATIONS

Everyone is empowered to Stop the Work whenever an unsafe situation is observed:
Stop Work authority should be reinforced
Any deviation to procedure should be submitted to a Management of Change and
risk assessment
Procedure to be thoroughly reviewed prior each operation to ensure that all steps
are properly defined and risk assessed
Ensure storage area even temporary is barried off




Final Issuance
The remaining J-Tube sections coming from the fabrication yard were transferred
directly from truck to construction vessel without temporarily storage along quayside
as per initial procedure
TOFS were held to review main findings of the incident
Initial investigation process started (ToR, Witness statement, storyboard…)


