Entry with Release Forms
Submission Instructions: Just 4 easy steps!
1. You (student) and your parent/guardian complete the Entry with Release Forms and give it to
your teacher.
2. Your teacher completes his/her section of the Entry with Release Forms.
NOTE: All entries MUST include the Entry with Release Forms, with all appropriate signatures of approval.
3. Give your teacher or parent/guardian your completed art piece OR link to the video you have
uploaded (to SchoolTube, Vimeo, or YouTube).
NOTE: You, your teacher, or parent/guardian will need to have a SchoolTube, Vimeo, or YouTube account to upload
the video. Please allow for extra time to set up an account if they do not already have one.
4. Your teacher or parent/guardian submits the Entry with Release Forms on your behalf along
with your entry file(for images) or link to the video.

** NOTE: Entries are preferred to be linked, scanned- or photographed- and emailed with your
release forms. IF your entry is a picture, poem, written song, painting, (anything else that is an
image or physical paper)… you may mail your entry and release forms to

National Creative Expression Contest
Verification of Authenticity
By entering The Be Kind People Project™ National Creative Expression Contest, I give The Be Kind People
Project permission to publish my work on their website, in other media, and for any purposes. I also
authorize the use of my photograph (if applicable). I understand that my work and my photograph may be
supplied to media outlets, which may elect to publish my work as well. I understand that I will not be paid
for the use of my work. All files and submissions, including any photos taken of me, will constitute the
property of The Be Kind People Project and may be used in publications, web publications and any other
media for purposes of this contest or related organizational activities. I retain all other rights to the work.
By signing the Verification of Authenticity and Entry with Release Form, the student, parent/guardian and
teacher/school agree to the terms set forth.
A completed Entry with Release Form is required with each individual student entry no later than Tuesday,
11:59pm PST, March 29, 2016. Student, Parent/Guardian, Teacher and Principal signatures are required.
We prefer to receive each entry via email along with the art files or video link. This is the quickest way to
process to the judges. However, if you must mail your entry, please refer to the submission instructions.
Winners will be announced by April 30, 2015.
If multiple students participate in the creation of the entry, a completed Entry with Release Form is
required for each student. If other students or adults, who are NOT official entrants, appear in the entry (i.e.
as “extras”), the teachers are responsible for obtaining approval of all participants in the entry. Teachers
are responsible for keeping those permission slips on file. The Be Kind People Project assumes no
responsibility for securing approvals for participants or entrants. Do not send appearance permission slips
(for “extras”) to The Be Kind People Project.
All video entries must adhere to the website usage rules of SchoolTube, Vimeo, or YouTube. Please see
www.bkpp.org for Official Rules.
