2015-16 Business Sponsorship Form
School/Organization Name provides opportunities for businesses to advertise while
supporting our school. Soon, we will host the Boosterthon Fun Run, a fun character and
fitness-focused fundraiser. Each student receives a customized Boosterthon t-shirt with
our school logo on the front. Students’ laps will be tallied on the back. In order to
provide all students with these school spirit shirts at no cost to their families, we are
partnering with local businesses like yours. Students wear their t-shirts year round as a
symbol of pride for their achievement. If you choose to be a sponsor, your business logo
will be displayed on the back of Insert Number of Students t-shirts.
Gold ($750) - Premium level on Boosterthon shirts, One Year Banner Contract, business
card on front page of website with link to your business for the school year and business
listed on each quarterly newsletter as a Gold Business Sponsor.
Silver ($500) –Middle level on Boosterthon T-shirts, One Year Banner Contract and
business listed on website.
Bronze ($250) – Lower level on Boosterthon T-shirts and business listed on Business
Sponsors page of PTO website.

Sponsorship Contract Terms
Banners shall be provided by the company sponsor and will be returned to the company
at the end of the contract term. In exchange for monetary consideration for the term,
your company’s banner will be hung on the fence at the entrance of the school. Banners
should not be larger than 4’ x 6’ (height by width). Depending on your sponsorship level,
the contract terms are provided above. The School/Organization Name is a 501(c)(3)
nonprofit organization. Our students, parents, and teachers appreciate your generosity
and support. Thank you!
