Eligibility Requirements


Homeless individual: Lacks a
fixed nightly residence and
has a behavioral health and/
or substance use diagnosis.
Sleeping on the street, couch
surfing, living in a location not
suited for a human to live, or
in a transitional facility.
Chronic homeless individual:
Must be homeless for a consecutive 12 month period or
4 times in 3 years equaling
12 months. Must have a
behavioral health and/or
substance use diagnosis.

Mountain HOME Team — Building A Bridge To HOME
Our Mission
Our goal is to provide services to
help underserved and homeless
individuals or families gain
independence and acquire a place
of their own; while learning
independent living skills
necessary for stable housing.
Who We Serve
The Mountain HOME Team program will
prioritize services first for homeless
veterans with a behavioral health and/or
substance use diagnosis and secondly,
chronically homeless individuals, families,
and/or youth who have behavioral health
and/or substance use diagnosis. While
these individuals and families are
prioritized, all homeless individuals with
behavioral health and/or substance use
diagnosis will be considered for the
program.
Services Provided
The Mountain HOME Team
provides therapy, peer support, case
management and comprehensive
community support services. We help
individuals and families find temporary and
permanent housing by linking them to
needed services focused on physical
health, employment, benefits and financial
resources.
