 Develop and implement composite permanency ranking scale that expands around the Casey
Family Programs ranking system (Composite Permanency Ranking System)
 Increase compliance with Statue 19-3-702(3.7) for court to consult with youth in ageappropriate manner regarding permanency plan
 Expand PRT process to include all OPPLA youth.
 Increase recruitment for alternative permanency placements and permanent connections
(including non-relative supports) by enhancing family finding and diligent search.
 Revise docket scheduling & standardize processes to increase docket efficiency, allowing for
sufficient time for meaningful permanency hearings.
 Increase compliance with statute 19-3-703 (Permanent Home Hearings).

The 2nd BPC will improve collaboration, communication and shared accountability by stakeholders to achieve better
outcomes with families.
1. To see all families in phase I every two weeks.
2. Attendance at staffing is mandatory for all treatment court professionals.
3. To train treatment court professionals on all assessments being used.
4. To develop a multiagency protocol of screening tools and assessments.
1. DHS will provide CFSR and social history to drug court MDT at initial review meetings for all cases being considered for
acceptance into drug court.
2. Develop a drug court “acceptance” criteria checklist by August 1, 2015.
3. Explore the GAIN and potential use for drug court.
4. Develop a policy on fair follicle testing by August 1, 2015.
1. Infuse 1 or more DANSR principles into all D&N cases that involve substance abuse in Huerfano by April 1, 2016.
2. 100% of identified parents and children who are involved in the 3rd JD D & N system shall have access to an effective
substance abuse and mental health assessment within 30 days of a shelter hearing.

3. The Best Practice Court Team will have an initial meeting on May 21, 2015 at 4PM (after the docket) in Walsenberg to
establish data for baseline collection and sharing of data (example - sharing of DSS safety and risk assessments).
4. Create a plan to obtain needed input from Mental Health and trauma treatment service providers to the BPC Team.
5. The BPC Team will meet monthly on docket days following completion of the docket (for a minimum of four months)
to collect and analyze all data and establish plan moving forward

1. Reduce the length of time between termination and adoption to less than 12 months for 60% of our adoption cases.

1. Continue to infuse family drug court principles into traditional D&N cases, consistent with the DANSR objectives and
timelines.
2. The 11th JD goal is to full engage court participants by minimizing perceived threats, avoiding re-traumatization and
support recovery through the recognition and awareness of the impact of trauma.
1. Reduce the number of terminations (by how much will be determined when data is pulled and analyzed).
2. Short term goal by July 2015: Standard operating procedures for all roles in the D&N court process. Documentation
requirements for shelter hearings and permanency planning hearings.

The team will set several aspirations for the next year including:

1. Continue to refine its implementation of family drug court practices into dependency and neglect cases to improve
outcomes for children and families in a VERY RURAL community.
2. Grand County will compile data to better understand the population impacted by dependency and neglect cases, the
needs of that population, and the outcomes achieved in dependency and neglect cases.

To complete FTC contract implementing FTC practices program goals. The success of this program will be measured by
looking at whether appropriate services were providing in a timely manner. Tammy will develop the goals by May 20,
2015. The team will set up a fatigue/vicarious trauma training.
1. Rework the 15th Judicial District Plan to overlap with the 16th. Also work permanency into it, it is not currently being
addressed.
2. Incorporate DANSR into court. We may begin a process even if we are not selected to be a DANSR pilot site.
3. No fault discussion, no fault with speed up the process. This would be to the departments use. The 15th is currently
getting more cases with no fault.

All 20th BPC team members will create a framework for incorporating parent and child input in development of treatment
plans to gain family buy-in and increase successful completion of treatment plan by November 30, 2015 through the
following actions:
 By May 31, 2015, BPC will hold a meeting to introduce goal to other BPC team members and choose subcommittee members.
 By May 31, 2015, BPC team members will identity other partiers for participation on sub- committee.
 BPC team members will identify stakeholders and key issues for timely starting treatment.
All FTDC team members will re-evaluate MHP assessments and FITC survey tool to identify FITC participants, matching
services to family needs by 12/31/2015.
1. Expedite permanency for youth in our jurisdiction.
2. Permanency will be achieved within 12 months of the initial out-of-home placement/removal. This is a “P Home Pilot
goal” and once a permanent home is identified then it will not be changed without clear and convincing evidence
emerges that it needs to be changed.
3. Reduce the time it takes the Court to establish permanency or return children home and thereby reduce the length of
cases
4. Improve communication of information by revamping the Family Service Plans to enhance management of clients
5. Obtain for information and training on DANSR (Dependency and Neglect System Reform Program)
6. Improve assessments as a means to improve services.
To establish a steering committee and meet for the first time by July 1, 2015. The committee membership will include at
minimum, members of the districts BPC/FTDC teams, the county attorney and the director of human services.
