I hereby give permission for my child to participate in the Northstar Soccer Camp with Northstar Church
on August 7-11, 2017. I hereby give Northstar Church personnel the authority to act on my behalf in the
case of an emergency and authorize treatment for my child if necessary and if I cannot be reached (parent
will be notified). I understand that I am financially responsible. I hereby release Northstar and its
personnel for all claims and for damages arising from any accidents or injury caused by my child’s
participation. I have also read and understood the information included in this brochure about activities
of religious nature that my child will be invited to participate in. I understand that my child may be
photographed for the promotion of Northstar Soccer Camp and give permission to Northstar Church to
use my child’s photo in promotional material. I understand that I will receive periodic emails from
Northstar about soccer camp and other kid’s events happening at the church.

Northstar Soccer Camp is sponsored by NORTHSTAR CHURCH. As a
church operated camp, there will be activities of a religious nature
each day, which includes but is not limited to Bible stories, Bible verse
memorization, and singing. We believe that it is critically important for
every person to have a personal relationship with Jesus Christ. As
such, participants may be invited to enter into such a relationship and
camp staff may personally talk and pray with participants. Signing the
registration form indicates you are in agreement with these activities.
Parents - Please take note of the following:
- The Camp will take place at the QYSA Indoor/Outdoor Soccer Facility (980 Anderson Dr.)
- Children will be given a Soccer Camp T-shirt that we would like them to wear to camp each day. This
may require washing them. Children will be given a water bottle that will be filled up for them each day.
- Children will be given a Northstar Soccer Camp soccer ball on the last day of camp, and a team photo
at the Family BBQ on Sunday, Aug. 13!

Be respectful of QYSA grounds and equipment.
Ensure your child is dressed for the weather including sunscreen/hat. Your child will be getting wet
at camp.
There is a hospitality area for parents. If you wish to stay at the field with your child we ask that you
stay in this area. This is for security reasons; we cannot have people wandering around the
facilities while we are trying to keep an eye on all your children.

Be respectful of all volunteers. Let your children know that they also need to be respectful of all
volunteers.
